准分子激光联合自拟消白方剂治疗白癜风的疗效及机制初探  

Influence of 308nm Excimer Laser Combined with White Prescription Treatment on Oxidative Stress and Inflammatory Factors in Patients with Vitiligo and the Curative Effect

摘　　要：目的观察308nm准分子激光联合自拟消白方剂治疗白癜风的疗效及对患者氧化应激反应及炎性因子的影响。方法将124例白癜风患者随机分为观察组(n=62例)和对照组(n=62例),对照组给予308nm准分子激光治疗,观察组在对照组治疗基础上给予自拟消白方剂治疗,两组疗程均为12周;观察两组治疗前后病变皮肤色素积分、皮肤病生活质量指数(DLQI)及两组治疗的疗效。检测两组治疗前后氧化应激反应相关指标过氧化氢酶(CAT)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSHPX)及丙二醛(MDA)水平,血清炎性因子白介素-12(IL-12)、白介素-17(IL-17)及可溶性细胞间黏附分子-1(s ICAM-1)水平。结果观察组治疗后病变皮肤色素积分显著提高,DLQI评分降低,有效率差异无统计学意义(P>0.05);两组治疗后血清CAT,SOD,GSH-PX水平显著提高,MAD水平显著降低(P均<0.05),观察组上述指标的改善情况均优于对照组(P均<0.05),且观察组治疗后上述指标与健康组差异无统计学意义(P均>0.05),而对照组与健康组差异有统计学意义(P均<0.05)。两组治疗后血清IL-12,IL-17及s ICAM-1水平显著降低(P均<0.05),观察组上述指标的改善情况均优于对照组(P均<0.05),且观察组治疗后上述指标与健康组差异无统计学意义(P均>0.05),而对照组与健康组差异有统计学意义(P均<0.05)。两组治疗期间均未发现严重不良反应。结论在308nm准分子激光治疗的基础上,应用自拟消白方剂能够显著抑制白癜风患者的氧化应激反应及炎性因子,改善临床症状,使得患者获得最佳临床疗效,且安全可靠,值得临床推荐。Objective To observe the curative effect and Influence of 308nm excimer laser combined with white prescription treatment on oxidative stress and inflammatory factors in in patients with vitiligo. Methods One hundred and twenty four patients with vitiligo were randomly divided intoobservation group（n=62 cases）and control group（n=62）. Control group was given 308nm excimer laser treatment, observation group was treated in the control group to give the white elimination on the basis of proprietary treatment, two groups of a course of 12 weeks. Observe two groups before and after treatment the lesion skin pigment integral, dermatology life quality index（DLQI）and the therapeutic effect of two groups. Detection of two groups before and after treatment oxidative stress related indicators of catalase（CAT）, superoxide dismutase（SOD）, glutathione peroxidase（gsh-px）and malondialdehyde（MDA）level, serum inflammatory factor, interleukin 12（IL- 12）, interleukin- 17（IL- 17）and soluble intercellular adhesion molecule 1（sICAM 1）levels. ResultsThe total effective ratehas no statistical difference between control and observation groups（P〉0.05）; Lesions in the two groups after treatment significantly improve skin pigment integral, DLQI score, the score improvement of observation group were better than the control group（P〈0.05）; Two groups after treatment serum levels of CAT, SOD, gsh-px levels increased significantly, MAD were significantly reduced（P〈0.05）, improve the index of the observation group were better than the control group（P〈0.05）, and the health of observation group after treatment group no statistical difference（P〉0.05）, while the control group and healthy group, all statistically significant（P〈0.05）.
