摘 要:目的分析2型糖尿病(T2DM)患者肺功能变化及与氧化应激反应的相关性。方法回顾性分析2014年1月-2016年12月上海市第一人民医院宝山分院呼吸科收治的T2DM患者80例(T2DM组)的临床资料,根据血糖水平、是否合并并发症,将T2DM组患者进一步分为轻度亚组(32例)、中度亚组(27例)和重度亚组(21例),有并发症亚组(42例)、无并发症亚组(38例)。另选取同期健康体检者80例作为健康对照组。检测各组对象肺功能及氧化应激指标变化,并分析二者的相关性。结果与健康对照组比较,T2DM组FVC、FEV_1及FEV_1/FVC均明显降低,MDA含量明显增加,SOD、GSH-PX活性明显降低(P<0.01);随着病情严重程度的增加,T2DM组肺功能明显降低,MDA含量明显增加,SOD、GSH-PX活性明显降低(P<0.05)。与无并发症患者比较,有并发症患者FVC、FEV_1及FEV_1/FVC均明显降低,MDA明显升高,SOD、GSH-PX活性明显降低(P<0.05)。T2DM患者FEV_1、FEV_1/FVC与各氧化应激指标均具有显著相关性(P<0.05)。结论肺损害是T2DM患者慢性并发症之一,且与病情严重程度相关,氧化应激反应可能参与了肺损害过程。Objective To explore the change of lung function in patients with type 2 diabetes mellitus(T2 DM).and analyze its correlation with oxidative stress. Methods Eighty patients(T2 DM group) with T2 DM who were admitted in our hospital from 2014 Jan to 2016 Dec were retrospectively analyzed. According to blood sugar levels, whether merger complications, T2 DM group was further divided into mild( n =32), moderate( n =27) and severe( n =21), with complications(n = 42),without complications( n =38). The indicators of lung function and oxidative stress were detected, and the correlation between them was analyzed. Results Compared with control group, the levels of FVC, FEV_1, FEV_1/FVC in T2 DM group were significantly reduced,MDA content was increased significanlly, activity of GSH-PX and SOD were reduced( P <0.05); With the increase of disease severity, lung function in T2 DM group was decreased significantly, MDA content was increased significantly, activity of GSH-PX and SOD were reduced( P <0. 05). Compared with that without complications, the levels of FVC, FEV_1, FEV_1/FVC in T2 DM group with complications were significantly reduced,MDA content was increased significantly, activity of GSH-PX and SOD were reduced( P <0. 05); FEV,, FEV_1/FVC in T2 DM patients was significantly associated with all the indicators of oxidative stress, FVC and GSH-PX was significantly positively correlated( P <0.05).Conclusion T2 DM lung impairment is an chronic complication of T2 DM, and associated with disease severity, oxidative stress may be involved in process of lung impairment.
