盐酸右美托咪定注射液复合盐酸罗哌卡因注射液用于腘窝坐骨神经阻滞的疗效观察

摘　　要：目的:观察盐酸右美托咪定注射液(以下简称右美托咪定)复合盐酸罗哌卡因注射液(以下简称罗哌卡因)用于腘窝坐骨神经阻滞的疗效。方法:80例成人择期拇外翻手术患者随机分为4组,各20例,超声引导下腘窝坐骨神经阻滞。R组:注射0.4%罗哌卡因20 mL,RD_(1~3)组:注射右美托咪定复合注射0.4%罗哌卡因20 mL。右美托咪定剂量:RD_1组为1.0μg·kg^(-1),RD_2组为1.5μg·kg^(-1),RD_3组为2.0μg·kg^(-1)。记录感觉阻滞和运动阻滞起效时间、维持时间及心动过缓、呼吸抑制等不良反应的发生情况。结果:感觉神经和运动神经阻滞:4组起效时间无明显差异(P>0.05);RD_(1~3)组持续时间延长明显长于R组(P<0.01),RD_2组和RD_3组明显长于RD_1组(P<0.05),RD_3组明显长于RD_2组(P<0.05);RD_3组镇静过度、心动过缓发生率明显高于R组、RD_1组、RD_2组(P<0.05)。结论:右美托咪定复合罗哌卡因用于腘窝坐骨神经阻滞时的右美托咪定的疗效以1.5μg·kg^(-1)剂量为适宜。Objective： To investigate the effects of dexmedetomidine hydrochloride injection combined with ropivacaine hydrochloride injection for treatment of popliteal sciatic nerve block. Methods： 80 adult patients undergoing elective hallux valgus surgery were randomly divided into 4 groups,each of 20 cases,ultrasound guided popliteal sciatic nerve block. Group R： 0. 4% ropivacaine 20 mL,group RD（1~3）： Dexmedetomidine injection ＋ injection 0. 4% ropivacaine 20 mL,dexmedetomidine dose,RD1 group was 1. 0 μg·kg^-1,group RD2 was 1. 5 μg·kg^-1,group RD3 was 2. 0μg · kg^-1. The effects of sensory block and motor block on the onset time,maintenance time,bradycardia and respiratory depression were recorded. Results： Sensory nerve and motor nerve block： there was no significant difference in the onset time between the 4 groups（ P〈0. 05）. The duration of RD（1~3） group was significantly longer than that of group R（ P〈0. 01）; group RD2 and group RD3 were significantly longer than group RD1（ P〈0. 05）; group RD3 was significantly longer than group RD2（ P〈0. 05）. The incidence of sedation and bradycardia in group RD3 was significantly higher than that in group R,RD1 and group RD2（ P〈0. 05）. Conclusion： Dexmedetomidine combined with ropivacaine for sciatic nerve block of the popliteal efficacy of dexmedetomidine with 1. 5 μg·kg^-1 dose is appropriate.
