经皮椎间孔镜术与椎板开窗髓核摘除术治疗腰椎间盘突出症的临床研究

摘　　要：目的:探讨经皮椎间孔镜术与椎板开窗髓核摘除术治疗腰椎间盘突出症的临床效果。方法:选取2013年9月-2016年12月在本院接受治疗的腰椎间盘突出症患者102例,按照手术方式分为传统组52例和PTED组50例,传统组采用椎板开窗髓核摘除术治疗,PTED组采用经皮椎间孔镜术治疗。比较两组VAS评分、OSWESTRY功能障碍指数评分及Lehmann腰椎功能评分、Macnab疗效、手术切口长度、出血量、手术时间、卧床时间、住院时间及并发症情况。结果:术后及术后3个月,两组VAS评分、OSWESTRY功能障碍指数评分、Lehmann腰椎功能评分、优良率及并发症发生率比较,差异均无统计学意义(P>0.05)。两组手术切口长度、出血量、手术时间、卧床时间、住院时间比较,差异均有统计学意义(P<0.05)。结论:经皮椎间孔镜术与椎板开窗髓核摘除术在腰椎间盘突出症患者治疗中疗效相似,但经皮椎间孔镜术术中对患者损伤小,利于患者快速恢复,但学习曲线较陡。Objective：To explore the clinical effect of percutaneous transforaminal endoscopic surgery and fenestration discectomy in the treatment of lumbar disc herniation.Method：102 patients with lumbar disc herniation were treated in our hospital from January 2015 to December 2016 were selected,according to the operation mode,they were divided into 52 cases in traditional group and 50 cases in PTED group,traditional group were treated with fenestration discectomy treatment,and PTED group underwent percutaneous transforaminal endoscopic surgery.The VAS score,OSWESTRY dysfunction index score and Lehmann lumbar function score,Macnab lumbar function effect,incision length,bleeding volume,operation time,bed time,hospitalization time and complications of two groups were compared.Result：The VAS score,OSWESTRY dysfunction index score,Lehmann lumbar function score,excellent rate and complication rate of two groups after operation and 3 months after operation were compared,the differences were not statistically significant（P〉0.05）.The incision length,bleeding volume,operation time,time in bed and hospitalization time of two groups were compared,the differences were statistically significant（P〈0.05）.Conclusion：Percutaneous transforaminal endoscopic surgery and fenestration discectomy have similar efficacy in the treatment of lumbar disc herniation,but percutaneous transforaminal endoscopic surgery in little injury to patients,help patients recover quickly,but the learning curve is steep.
