加速康复策略在腹腔镜下胃减容术后患者ICU监护中的应用

摘　　要：目的:探讨加速康复策略在腹腔镜下胃减容术后患者ICU监护中的应用。方法:回顾性分析2011年1月~2017年5月在我院行腹腔镜下胃减容术后收住入ICU的20例肥胖患者的临床资料,在ICU期间均给予术后加速康复理念指导下早期目标导向的镇静镇痛治疗、有效循环和呼吸管理、早期康复锻炼、完善过渡期护理。结果:20例患者均顺利脱机拔管,未发生手术相关并发症,生命体征平稳,转入普通病房继续治疗。通过随访,20例患者均恢复良好,顺利出院。结论:腹腔镜胃减容术治疗病态性肥胖效果好,而术后加速康复策略在患者转入ICU期间的应用是确保手术顺利进行、减少术后并发症,促进术后良好恢复的关键。Objectives To discuss the application of accelerated rehabilitation strategy in ICU monitoring after laparoscopic gastric volume reduction surgery.Methods：A retrospective analysis of the clinical data of 20 obese patients who received laparoscopic gastric volume reduction surgery and were admitted to ICU in our hospital from January 2011 to May 2017.They received sedative and analgesic therapy,effective circulatory and respiratory management,early rehabilitation exercise and perfect transition period nursing under the guidance of the concept of accelerated rehabilitation when they were treated in ICU.Results：All of the 20 patients were successfully removed from the breathing machine without complications.With stable vital signs,they were transferred to the general ward for further treatment.Through follow-up,20 patients recovered well and were discharged smoothly.Conclusion：Laparoscopic gastric volume reduction surgery has good treatment effect on morbid obesity.The application of accelerated postoperative rehabilitation strategy during ICU is critical for smooth operation,reduction of postoperative complications,and promotion of good postoperative recovery.
