罂粟碱配合护理干预预防断指再植术后血管痉挛30例效果观察

摘　　要：目的探讨罂粟碱配合护理干预预防断指再植术后血管痉挛的效果。方法选择2013年11月至2017年1月在医院行断指再植术的患者60例,按随机数字表法分为观察组与对照组,各30例。对照组患者给予断指再植术常规治疗及临床常规护理路径,观察组患者在对照组基础上给予罂粟碱配合护理干预。结果干预前,两组患者疼痛评分对比无差异(P>0.05);干预后,观察组患者术后7,14 d时的疼痛评分、毛细血管充盈时间、患指与健指温度差及血管痉挛、血管栓塞等血管危象的发生率均较对照组低,血氧饱和度(Sp O2)水平、断指再植成功率则显著高于对照组(P<0.05)。结论罂粟碱配合护理干预能维持断指再植患者良好的患指皮肤温度,缩短毛细血管充盈时间,增加Sp O2,改善患指血供,降低术后血管痉挛的发生风险。Objective To observe the effect of papaverine combined with nursing intervention in the prevention of postoperative vasospasm in patients with replantation of severed fingers. Methods Totally 60 patients treated with replantation of severed fingers in our hospital from November 2013 to January 2017 were divided into the observation group and the control group according to the random number table method,30 cases in each group. The control group was given replantation of severed fingers routine treatment and clinical routine nursing pathway, on this basis, the observation group was given papaverine combined with nursing intervention. Results There was no difference in VAS scores between the two groups before treatment（ P〉0. 05）. After intervention, the VAS scores, the time of capillary filling,the temperature difference between the skin of affected fingers and healthy fingers,the incidence rates of vascular crisis such as vasospasm and vascular embolization in the observation group were lower than those in the control group at 7 d and 14 d after operation,while the level of Sp O2 and the success rate of replantation were significantly higher than those in the control group,and the difference between the two groups was statistically significant（ P〈0. 05）. Conclusion Papaverine combined with nursing intervention on the basis of conventional treatment nursing can maintain good finger skin temperature,shorten the filling time of the capillaries,increase Sp O2 and blood supply,and reduce the risk of postoperative vasospasm in patients with replantation of severed fingers.
