3213例生殖道支原体结果分析及临床送检意义探讨

摘　　要：目的对支原体培养计数及药敏结果耐药率分析,指导临床合理使用抗生素及临床送检意义探讨。方法整群选取该院2011年1—12月门诊患者,男性946例,女性2 267例,以初诊为尿道炎、前列腺炎、宫颈炎或不孕不育而行支原体培养和药敏试验。应用法国梅里埃公司生产的Mycoplasma IST 2解脲支原体(U.urealyticum)和人型支原体(M.hominis)培养鉴定、计数及药敏试验试剂盒。结果女性患者感染率为80.2%,明显高于男性的19.8%,两者相比差异有统计学意义(P<0.01)。Uu+Mh混合感染者对交沙霉素、强力霉素及四环素的敏感率较高,依次为94.1%、89.6%、78.7%;Mh感染者也是对交沙霉素、强力霉素及四环素的敏感率较高,依次为97.7%、97.7%及88.4%。Mh及Uu+Mh混合感染者的耐药率明显高于Uu感染者。四环素类强力霉素和大环内酯类的交沙霉素的敏感率较高,在93.0%及98.0%以上,可作为该院治疗支原体感染的首选药物。结论泌尿生殖道分泌物支原体培养的高阳性率但绝大多数仍可能只是携带者,可随着抗生素的广泛应用和不合理用药以及患者的自行用药,使得耐药率不断增加,临床广泛应用支原体培养法值得商榷。Objective To analyze the mycoplasma bacteria count and drug sensitivity results tolerance rate thus guiding the clinical rational use of antibiotics and studying the clinical submission significance. Methods Group selection 946 males and 2 267 females in our hospital from January to December 2011 were selected and these patients were initially diagnosed with urethritis, prostatitis, cervicitis or infertility and they were given the mycoplasma culture and drug sensitivity test by the Mycoplasma IST 2 U.urealyticum and M.hominis produced by France Merieux Company and drug sensitivity test kit.Results The infection rate of female patients was obviously higher than that of male patients,(80.2% vs 19.8%), and the difference was statistically significant(P<0.01), and the sensitivity rate of Uu+Mh mixed infection patients to the josamycine,doxycycline and polycycline was high, which was respectively 94.1%,89.6%,78.7%, and the sensitivity rate of Mh infection patients to the josamycine, doxycycline and polycycline was high, which was respectively 97.7%,97.7% and 88.4%, and the drug-tolerance rate of Mh and Uu+Mh mixed infection patients was obviously higher than that of Uu infection patients,and the sensitivity rates of doxycycline in tetracyclines and josamycine in macrolides were higher, respectively above 93.0% and 98.0%, which can be used as the first choice in treatment of mycoplasma infection Conclusion The positive rate of urogenital tract secretions mycoplasma culture is high but most of them may only be the carriers, with the wide application and irrational drug use and self-medication of patients, the drug tolerance rate is constantly increasing, and whether the mycoplasma culture method can be widely used in clinic is worth consideration.
