腹腔镜疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的临床效果比较分析

摘　　要：目的分析腹腔镜疝修补术与开放式无张力、疝修补术治疗成人腹股沟疝的临床效果比较。方法方便选择2016年1月—2017年4月收治的76例成人腹股沟疝患者根据随机数字表法分组,分别为开放式无张力组(实施开放式无张力手术)、腹腔镜疝修补组(实施腹腔镜疝修补手术),各有38例,比较手术效果。结果腹腔镜疝修补组腹股沟疝治疗效果和开放式无张力组均94.74%差异无统计学意义(P>0.05);腹腔镜疝修补组卧床时间(12.13±2.91)h、疼痛维持时间(2.26±0.13)d、住院时间(5.26±0.11)d短于开放式无张力组(20.14±1.15)h、(3.52±0.42)d、(7.51±0.12)d,手术费用(9 424.98±672.13)元高于开放式无张力组(6 414.02±561.01)元(P<0.05);两组切口血肿、尿潴留等并发症发生率相近,其中,两组均分别有1例切口血肿、1例阴囊血肿和1例尿潴留,发生率均为7.89%(P>0.05)。结论腹腔镜疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的临床效果相似,但腹腔镜疝修补术可缩短术后康复时间和减轻疼痛,但手术费用较高,可结合患者经济情况选择合适的手术。Objective This paper tries to compare the clinical effect of laparoscopic hernia repair and open tension-free hernia repair in the treatment of adult inguinal hernia. Methods From January 2016 to April 2017 a total of 76 patients with adult inguinal hernia were divided into two groups according to the random number table method: the open tension-free group(adopting open tensionless operation), the laparoscopic hernia repair group(adopting laparoscopic hernia repair surgery), with 38 cases in each group, the surgical results were compared. Results There was no significant difference in the treatment of inguinal hernia with laparoscopic hernia repair and open tension-free hernia repair, both were 94.74%(P >0.05). The bedtime time was(12.13 ±2.91)h and the pain retention time was(2.26 ±0.13)d, hospitalized time was(5.26 ±0.11)d in the laparoscopic group, shorter than those in the open tension-free group of(20.14 ±1.15)h,(3.52 ±0.42)d,(7.51±0.12)d, and the cost of surgery was(9 424.98±672.13) Yuan, lower than the open tension-free group of(6 414.02±561.01) Yuan(P<0.05); the complications of wound hematomas and urinary retention in the two groups were similar, there was 1 cases of incisional hematoma, 1 case of scrotal hematoma and 1 case of urinary retention in the two groups, the incidence rate was 7.89%(P >0.05). Conclusion Laparoscopic hernia repair has the similar treatment with the open tension-free hernia repair for adult inguinal hernia. However, laparoscopic hernia repair can reduce postoperative recovery time and reduce pain and the cost of surgery is high. Patients can choose the appropriate surgery combined with their economic situation.
