简易鼻塞式CPAP治疗新生儿呼吸困难的护理

摘　　要：目的总结简易鼻塞式CPAP(持续正压通气)治疗新生儿呼吸困难情况的护理要点。方法方便选取该院于2015年2月—2016年4月间接收的56例新生儿呼吸困难患儿,均采用简易鼻塞式CPAP治疗,取得家长知情同意后按照便利抽样法将患儿划分为常规组28例与观察组28例,常规组患儿提供一般护理服务,观察组患儿提供综合护理干预,评价两组患儿干预后SpO_2(血氧饱和度)、FiO_2(吸入氧浓度)值,评估患儿家长对护理质量评分。结果常规组患儿SpO_2值明显低于观察组;观察组患儿FiO_2值明显低于常规组(P<0.05)。常规组患儿家长对病情观察评分为(18.23±1.02)分、呼吸监测评分为(17.28±1.00)分、用药监督为(17.18±1.00)分、皮肤护理评分为(16.58±0.91)分;观察组患儿家长对病情观察评分为(22.31±0.55)分、呼吸监测评分为(23.10±0.34)分、用药监督为(22.49±0.21)分、皮肤护理评分为(23.02±0.19)分,观察组家长对各项护理质量评分结果均优于常规组(P<0.05)。结论科室为呼吸困难新生儿提供简易鼻塞式CPAP治疗,同时为患儿提供综合护理干预措施,可有效改善患儿呼吸困难表现,提高科室护理质量。Objective This paper tries to observe the clinical effect of nursing intervention of the simple nasal CPAP(continuous positive airway pressure) in the treatment of neonatal dyspnea. Methods 56 neonates with dyspnea treated from February 2015 to April 2016 in this hospital were convenient selected and adopted the simple nasal CPAP treatment. With the family members' consents, the subjects were randomly divided into two groups, with 28 cases in each group. The conventional group adopted the general nursing care; the observation group adopted the comprehensive nursing care. After intervention, the indexes of SpO_2 and FiO_2 as well as the family member's evaluation scores of the two groups were recorded.Results The index of SpO_2 in the conventional group was significantly lower than that in the observation group; the index of FiO_2 in the observation group was significantly lower than conventional group(P <0.05); the family member's evaluation scores of disease observation, respiration monitoring, clinical supervision and skin treatment for the conventional group were(18.23±1.02)points,(17.28±1.00)points,(17.18±1.00)points and(16.58±0.91)points; and those for the observation group were(22.31 ±0.55)points,(23.10 ±0.34)points,(22.49 ±0.21)points and(23.02 ±0.19)points; the family member's evaluation scores for the observation group were higher than the conventional group( P <0.05). Conclusion For neonates with dyspnea, the simple nasal CPAP and comprehensive nursing care can effectively improve the clinical symptoms and nursing quality.
