25G细针腰麻在老年患者麻醉中的应用分析

摘　　要：目的探讨25G细针腰麻在老年患者麻醉中的应用有效性与安全性。方法方便选取2015年6月—2016年12月该院收治的120例老年下肢手术患者为研究对象,依据麻醉方式分为实验组(25G细针腰麻,60例)和对照组(腰硬联合麻醉,60例)。对比观察两组麻醉完成时间、感觉阻滞起效时间、术中麻醉效果、术中血压、心率及术后并发症。结果实验组麻醉完成时间(137.8±29.6)s,术后神经损伤10.0%,腰背痛13.3%,头痛6.7%,与对照组比较差异有统计学意义(P<0.05)。实验组术中不同时刻,患者心率、血压无明显波动。结论 25G细针腰麻用于老年麻醉具有操作方便、起效快、效果确切、循环稳定、并发症少的优点,具备临床推广价值。Objective This p a p e r tries to inves tig ate th e efficacy an d safety of 2 5G fin e n e e d le lum b a r a n e s th e s ia in eld e rly patients. Methods C onvenient se le c t io n 120 c a se s of sen i le lower extremity surgery from Ju n e 2 01 5 to Dec em b er 2 01 6 were selected as the research objects and divided into the experimental group （25G fine needle lumbar anes thesia, 60 cases） and the control group （lumbar hard joint anes thesia, 60 cases） according to anesthesia methods. The duration of anesthesia com-plet ion, sensory block onset time, intraoperative anes thesia, intraoperative blood pressure, hear t rate and postoperative com-plicat ions were observed and compared between the two groups. Results T he an e s th e t ic comp let io n time of th e ex p erim en ta l group was （137.8±29.6） s, pos toperative nerve injury of 10.0%, backache of 13.3%, headache of 6.7%, with s ignif icant differ-ence compared with the control group（P〈0.05）. There was no signif icant fluctuation of hear t rate and blood pressure in the ex-per imental group at different time. Conclusion T he 2 5G fine n e e d le lum b a r a n e s th e s ia has th e ad v an tage s of co n v en ien t op-erat ion, quick effect, accurate result , s table circulat ion and fewer complicat ions, and has the value of cl inical popularization.
