经鼻导管湿化高流量吸氧在ICU患者撤机后的应用效果分析

摘　　要：目的对ICU患者撤机后实施经鼻导管湿化高流量吸氧方案的临床价值进行评定。方法研究从2014年1月—2017年5月间在该院ICU治疗且已撤机的患者中,方便选取出75例研究,以费雪派克湿化器对其实施经鼻导管湿化高流量吸氧方案,观察其治疗后1、8 h、24 h的呼吸指数、氧合指数以及二氧化碳分压指数等,并且对其气道组织湿化状况、舒适度以及耐受程度等进行评定。结果 75例患者的治疗后1 h呼吸指数(20.38±1.14)次/min,治疗后8 h是(22.87±2.26)次/min,治疗后24 h是(22.68±1.69)次/min。同时,患者气道组织湿化程度为(2.19±0.19)分,舒适度是(1.68±0.28)分,耐受程度为(1.19±0.17)分。结论对于完成撤机操作的ICU患者,以费雪派克湿化器对其实施经鼻导管湿化高流量吸氧方案可控制其呼吸指数、氧合指数以及二氧化碳分压指数,且保证患者气道组织湿化水平、舒适度以及耐受程度,可推广。Object ive This paper tries to evaluate the cl inical value of the nasal catheter wetting and high flow oxygen in the ICU pat ients af ter weaning. Methods 75 cases of ICU pat ients af ter weaning in this hospi tal from January 2014 to May 2017 were convenient selected as the research objects and were treated with f isher parker humidif ier by nasal catheter wet-ting and high flow oxygen therapy, and then the breathing index, oxygenation index and carbon dioxide par t ial pressure index af ter 1 h, 8 h, 24 h treatment were observed, and the wetting condi tion of airway t is sue, comfort degree, and the degree of tolerance were assessed. Results In the 75 pat ients, the breath index af ter 1h treatment was （20.38±1.14） times/min,and that of 8 h was （22.87±2.26）times/min, and that of 24 h was （22.68±1.69）times/min. At the same time, the degree of moisture in the pat ient ’s airway t is sue was（2.19±0.19）points, and the comfort level was （1.68±0.28）points, and the tolerance level was （1.19土 0.17）points. Conclusion For the ICU pat ients who completed the weaning operat ion, the f ischer parker humidif ier was used to control the respiratory index, oxygenation index and carbon dioxide par t ial pressure index of the nasal catheter wetting and high flow oxygen regimen, and to ensure that the pat ient ’s airway t is sue wetting level, comfort and tolerance level, so it is worth promotion.
