吸入糖皮质激素联合小剂量茶碱 白三烯调节剂治疗5岁以下儿童哮喘的临床研究

摘　　要：目的探讨吸入糖皮质激素联合小剂量茶碱、白三烯调节剂治疗5岁以下儿童哮喘的临床效果。方法选取我院儿科住院部2016年4月—12月收治的符合中度持续性哮喘诊断的患儿120例,按照随机数字表分为对照组和观察组,每组60例。对照组通过面罩式雾化吸入药物丙酸氟替卡松,同时口服缓释茶碱片,观察组在对照组的基础上加服孟鲁司特,疗程均为6个月,比较2组患儿治疗前后哮喘控制测试(ACT)评分及日间和夜间症状评分。结果治疗后3个月及6个月,观察组患儿ACT评分明显高于对照组(P<0.05),观察组患儿日间症状评分和夜间症状评分明显低于对照组(P<0.05);治疗期间2组患儿不良反应发生年均为6.67%。结论吸入糖皮质激素联合小剂量茶碱、白三烯调节剂治疗5岁以下儿童哮喘临床效果显著,值得推广。Objective To investigate the clinical effect of inhaled corticosteroids combined with small doses of theophylline and leukotriene modulators in the treatment of asthma in children under 5 years of age.Methods 120 cases of children with moderate persistent asthma were enrolled in our department from April 2016 to December 2016.According to the random number table,the control group and the observation group were divided into 60 cases.The control group was treated with masked atomization of fluticasone propionate and oral sustained-release theophylline tablets.The observation group was treated with montelukast on the basis of the control group.The course of treatment was 6 months.ACT（Asthma Control Test） score and day and night symptom scores.Results The scores of ACT in the observation group were significantly higher than those in the control group at 3 months and 6 months after treatment（P〈0.05）.The daytime symptom score and nighttime symptom score of the observation group were significantly lower than those of the control group（P〈0.05）,There was no significant difference in the incidence of adverse reactions between the two groups（P〉0.05）.Conclusion Inhaled glucocorticoid combined with small dose of theophylline and leukotriene modifier in the treatment of children under 5 years old,the clinical effect is remarkable,it is worth promoting.
