基于正交试验设计的温和灸长强穴治疗肛门瘙痒症的最佳治疗方案研究

摘　　要：目的采用正交试验设计探讨温和灸长强穴治疗肛门瘙痒症的最佳治疗方案。方法选取2013—2014年浙江中医药大学附属第一医院肛肠科门诊收治的肛门瘙痒症患者32例,温和灸长强穴、肛周局部皮肤,以治疗前后瘙痒程度、瘙痒频率、生活质量评分为观察指标,采用3因素(A:温和灸时间,B:温和灸次数,C:温和灸疗程)2水平(排除各因素间的相互影响)正交设计制成L_42~3的正交表。按照正交试验设计表将患者随机分成试验1组、试验2组、试验3组、试验4组。结果治疗后4组瘙痒程度评分比较,差异无统计学意义(P>0.05);4组瘙痒程度评分治疗前后组内比较,差异有统计学意义(P<0.05)。治疗后4组瘙痒频率评分比较,差异无统计学意义(P>0.05);4组瘙痒频率评分治疗前后组内比较,差异有统计学意义(P<0.05)。治疗后试验1组生活质量评分高于试验3组(P<0.05);治疗后试验1组、试验2组生活质量评分高于试验4组(P<0.05);4组治疗前后生活质量评分组内比较,差异有统计学意义(P<0.05)。瘙痒程度评分影响因素的主次顺序为B>A=C,最佳治疗方案为20 min/次,2次/d,4周为1个疗程。瘙痒频率评分影响因素的主次顺序为A>B=C,最佳治疗方案为20 min/次,2次/d,2周为1个疗程。生活质量评分影响因素的主次顺序为A>B>C,最佳治疗方案为20 min/次,2次/d,2周为1个疗程。结论温和灸长强穴对肛门瘙痒症产生治疗作用的主要影响因素是温和灸的治疗时间,最佳治疗方案为20 min/次,2次/d,2周为1个疗程。Objective To determine the optimal treatment regimen for pruritus ani using mild moxibustion at Changqiang acupoint with moxa rolls based on the orthogonal design. Methods Thirty-two cases of pruritus ani received mild moxibustion at Changqiang acupoint and local perianal skin with moxa rolls in Anus Intestine Surgery Outpatient,the First Affiliated Hospital of Zhejiang Chinese Medical University from 2013—2014 were enrolled. According to the self-developed orthogonal array,L_42~3. Using 3 factors（ A： duration of mild moxibustion per time,B： times of mild moxibustion,C： length of the mild moxibustion course） and 2-level collocation（ elimination of the interaction efects between factors）, they were randomly divided into group 1, group 2, group 3 and group 4. The therapeutic effects in all groups were assessed from the improvement achieved in pruritus severity score,itching frequency,and Dermatology Life Quality Index score. Results After treatment,the pruritus severity score, frequency of itching and Dermatology Life Quality Index score were all significantly improved in each of the groups（ P 0. 05 for each）. The pruritus severity score and frequency of itching did not differ substantially between the groups after treatment（ P 0. 05 for each）. After treatment,the Dermatology Life Quality Index score in group 1 was higher than that in group 3（ P 0. 05）,and that was lower in group 4 compared with group 1 and group 2（ P 0. 05）. The pruritus severity score was affected mostly by factor B,and it was equally affected by factors A and C. The optimal treatment regimen for pruritus using mild moxibustion at Changqiang acupoint and local perianal skin with moxa rolls was： the duration of treament per time： 20 min; frequency of treament： twice a week; length of the course of treament： 4 weeks. The frequency of itching was mostly affected by factor A,and it was equally affected by factors B and C. The optimal regimen for reducing the frequency of itching using mild moxibustion at Changq
