摘　　要：目的探讨剪切波弹性成像(SWE)对桥本甲状腺炎(HT)与Riedel甲状腺炎(RT)的诊断价值。方法收集2014年5月—2016年11月于海南省人民医院行SWE检查并经手术病理证实的HT患者35例(HT组)及RT患者9例(RT组),选择同期于本院体检健康者30例作为对照组。对3组SWE弹性值均值进行统计学分析,以病理诊断为金标准,绘制SWE弹性值均值诊断两种甲状腺炎的受试者工作特征(ROC)曲线。结果对照组SWE弹性值均值低于HT组及RT组(P<0.01)。RT组SWE弹性值均值高于HT组(P<0.01)。SWE弹性值均值诊断HT的ROC曲线下面积为0.90[95%CI(0.86,0.95)],临界点为65.45 k Pa,灵敏度为88.9%,特异度为91.4%;SWE弹性值均值诊断RT的ROC曲线下面积为0.88[95%CI(0.83,0.92)],临界点为72.20 k Pa,灵敏度为82.9%,特异度为88.5%。结论 SWE对HT与RT具有较好的诊断价值,可以为临床医生提供重要的诊断依据。Objective To investigate the diagnostic performance of shear wave elastography（ SWE） for evaluation of Hashimoto＇s thyroiditis（ HT） versus Riedel thyroiditis（ RT）. Methods The enrolled participants were all from Hainan General Hospital,including 35 cases underwent SWE and were confirmedly diagnosed with HT pathologically（ HT group）,and 9 cases underwent SWE and were confirmedly diagnosed with RT pathologically（ RT group）,and 30 cases underwent physical examination（ control group） from May 2014 to November 2016. The mean elasticity values from SWE in all the groups were analyzed and assessed by the gold standard（ results of pathological examination）. ROC analysis was performed to investigate the diagnostic accuracy of SWE for HT and RT. Results Among the groups,the mean elasticity values for the control group were found to be the lowest（ P 0. 01）,while those for the RT group were the highest（ P 0. 01）. For the diagnosis of HT,the AUC of SWE was 0. 90,with a cut-off point of 65. 45 k Pa,sensitivity of 88. 89%,specificity of 91. 43%,respectively. For the diagnosis of RT,the AUC of SWE was 0. 88,with a cut-off point of 72. 20 k Pa,sensitivity of 82. 86%,specificity of88. 52%,respectively. Conclusion SWE possesses high diagnostic accuracy for both HT and RT,which can provide important diagnostic information for clinicians.
