男性不育患者生殖道溶脲脲原体感染对精液质量的影响

摘　　要：目的探讨男性不育患者生殖道溶脲脲原体感染对精液质量的影响。方法选取2016年8月-2017年3月医院所收治的168例男性不育患者作为不育组,另选同期成年男性志愿者150例作为已育组。比较分析两组生殖道感染溶脲脲原体情况,生殖道溶脲脲原体感染与非感染男性不育患者精液参数、精子活动能力及α-葡萄糖苷酶水平。结果不育组生殖道溶脲脲原体感染率35.12%显著高于已育组24.67%(P<0.05)。与溶脲脲原体阴性相比,溶脲脲原体阳性患者精子密度(85.58±9.74)×10~6/mL,精子活率(45.74±5.22)%,精子活力(39.55±4.36)%均明显降低,差异均具有统计学意义(P均<0.05);溶脲脲原体阳性患者精子平均曲线运动速度(34.08±4.75)μm/s,平均直线运动速度(23.40±3.02)μm/s,平均路径速度(27.08±3.57)μm/s,平均移动角度(47.81±5.83)°,平均鞭打频率(5.03±0.59)%与溶脲脲原体阴性相比均明显降低,差异均具有统计学意义(P均<0.05);溶脲脲原体阳性患者α-葡萄糖苷酶水平处于异常的比率为50.85%明显高于溶脲脲原体阴性患者29.36%(P均<0.05)。结论在男性不育中生殖道溶脲脲原体感染相对较高,并对精液参数和精子活动能力造成一定的影响,从而导致精液质量的大幅度降低,进而引起男性不育。OBJECTIVE To probe into and analyze the effect of Ureaplasma urealyticuminfections in genital tract on semen quality in male infertile patients.METHODS A total of 168 male infertility patients from Aug.2016 to Mar.2017 were selected in the hospital as infertile group.During the same time,150 adult male volunteer were selected as fertile group.The U.urealyticuminfection situation between the two groups,the physiological parameters of semen parameters,sperm motility andα-glucosidase levels in U.urealyticuminfected and non-infected male infertility patients were compared and analyzed.RESULTS The U.urealyticuminfection rate in genital tract of the patients in infertile group was 35.12%,which was significantly higher than that in fertile group（P〈0.05）.Compared with U.urealyticum negative patients,in the patients with U.urealyticum positive,the sperm motility was（85.58±9.74）× 10~6/mL,sperm viability was（45.74±5.22）% and sperm motility was（39.55±4.36）%,there were decreased significantly（P〈0.05）.In the patients with U.urealyticum positive,the curvilinear velocity was（34.08±4.75）μm/s,straight line velocity was（23.40±3.02）μm/s,average path velocity was（27.08±3.57）μm/s,movement angle was（47.81± 5.83）°and beat-cross frequency was（5.03±0.59）%and the differences were significant（P〈0.05）.In the patients with U.urealyticum positive,the normal rate ofα-glucosidase was 50.85%,which was significantly higher than that in patients with U.urealyticumnegative（P〈0.05）.CONCLUSION In the male infertility,the infection of U.urealyticumin the reproductive tract is relatively high,and the semen parameters and sperm motility are affected,which leads to a significant reduction in semen quality,causing male infertility.
