颈椎稳定性训练联合芍药木瓜汤治疗72例颈性眩晕

摘　　要：目的探讨颈椎稳定性训练联合芍药木瓜汤治疗颈性眩晕的临床疗效。方法选取我院诊断及治疗的颈性眩晕患者144例,按随机数字表法分组,对照组72例予以胞二磷胆碱注射液0.5 g+0.9%氯化钠注射液250 m L静脉静滴,维生素B6注射液100 mg+0.9%氯化钠注射液250 m L静脉静滴,研究组72例予以颈椎稳定性训练联合芍药木瓜汤(方药组成:白芍、木瓜、当归、川芎、白芷、丹参、杜仲、姜黄、甘草),2组共治疗4周,观察并记录2组间多普勒检测结果、血清CRP、ET-1、CGRP水平及血液流变学指标,同时对比临床疗效及并发症状况。结果对照组治疗有效率77.78%低于研究组的治疗有效率90.28%,具有统计学意义(P<0.05);与对照组比较,研究组治疗后LVA、RVA及VB平均血流速度较高,治疗后血清CRP、ET-1水平较低,CGRP水平较高,治疗后血液黏度指标及红细胞压积较低,具有统计学意义(P<0.05);2组间无恶性不良反应出现,仅出现胃肠道不良反应,对症处理后迅速缓解。结论采用颈椎稳定性训练联合芍药木瓜汤治疗颈性眩晕疗效确切,推测其可能机制为升高颈部血流速度,降低血液黏度。Objective To investigate the clinical effect of stability of cervical spine combined with traditional Chinese medicine in the treatment of 72 cases with cervical vertigo.Methods 144 cases of cervical vertigo from our hospital were selected,patients or family member signed an agreement,and patients were divided into 2 groups according to random number table.72 cases in the control group were treated with citicoline injection0.5g＋0.9% sodium chloride injection 250 ml iv,qd,vitamin B6 injection 100mg＋0.9% sodium chloride injection 250 ml iv,qd,72 cases in the experiment group were treated on the base of the control group with stability of cervical spine combined with Shaoyao Mugua decoction,composed of herbs： Radix Paeoniae Alba,papaya,angelica,Sichuan lovage rhizome,angelica,salvia,eucommia,turmeric,licorice,and 2 groups were treated for 4 weeks.Doppler test results,serum CRP,ET-1,CGRP levels and blood rheology indicators,clinical efficacy and complications were compared after the treatment.Results The effective rate in the control treatment group （77.78%） was lower than the experiment group（90.28%）,with significant difference（P0.05）,and compared with the control group,LVA,RVA and VB mean blood flow velocity in the experiment group were increased after the treatment,levels of serum CRP,ET-1,CGRP were higher,the index of blood viscosity and red blood cell hematocrit were lower,with statistical significance（P0.05）.Conclusion Stability of cervical spine combined with Shaoyao Mugua decoction in the treatment of cervical vertigo was effective and safety.
