摘 要:目的探讨Renyi排序熵(RPE)在小儿麻醉深度监测中的应用价值。方法选择手术治疗患儿22例,其中男性12例,女性10例;年龄1~12岁,平均年龄7岁。排除心脑血管疾病及呼吸系统疾病的病例。术前禁食、禁饮8 h。患儿入手术室后行静脉快速诱导全身麻醉,术中以静吸复合药物维持麻醉。在整个手术过程中,记录患儿的脑电图(EEG)信号,包括清醒期、诱导期、深度麻醉期和意识恢复期4个阶段,同时记录手术中的用药情况和各个时间点的脑电双频指数(BIS)值。结果在对不同指标的统计图中可以看出,同步快慢比(SFS)与β比率在区分清醒期和诱导期有较大的重叠,而RPE的重叠现象不明显。在对不同指标的相关性统计表中可以看到β比率、SFS与BIS的相关性在0.8以下,而RPE与BIS的相关性在0.83左右。结论 RPE对小儿麻醉深度监测具有很好的状态分辨能力,与BIS具有更高的相关性,RPE在小儿麻醉深度监测中有着潜在的应用价值。Objective To explore the application of pediatric anesthesia depth monitoring based on Renyi permutation entropy(RPE). Methods A total of 22 children with surgical treatment were enrolled, which included 12 males and 10 females, aged1-12 years old with mean age of 7 years old. The cases with cardiovascular and cerebrovascular diseases or respiratory diseases were excluded. All of them were fasted for 8-hour before surgery, then performed by general anesthesia with intravenous rapid induction at operating room and maintained by intravenous inhalational anesthesia in operation. During the whole operation, the electroencephalogram(EEG) signals(4 stages: awakening period, induction period, deep anesthesia period and con-sciousness recovery period) were recorded. At the same time, the medication situation and bispectral index(BIS) were recorded.Results In the graphs of different indicators, the synchronization speed ratio(SFS) and β ratio showed larger overlap between awakening period and induction period, while RPE overlap phenomenon was not obvious. In the correlation statistics of different indicators, the correlation between β ratio, SFS and BIS was below 0.8, and the correlation between RPE and BIS was about 0.83. Conclusion It is demonstrated that RPE showed good resolution ability for pediatric anesthesia depth monitoring,which has higher correlation with BIS. So RPE showed potential application value in pediatric anesthesia depth monitoring.
