排毒保肾丸对腹膜透析营养状况的影响

摘　　要：目的观察排毒保肾丸(大黄虫丸加减)对腹膜透析(PD)营养状况的影响。方法将符合研究要求的连续性不卧床腹膜透析(CAPD)患者80例随机分为治疗组和对照组,各40例。在基础治疗的同时,治疗组加用排毒保肾丸3 g,每日2次;对照组加用复方酮酸片0.12 g·kg-1·d-1,治疗6月后观察2组患者治疗前后白蛋白(Alb)、肌酐(Scr)、尿素氮(BUN)、标准化蛋白质呈现率(nPNA)、血红蛋白(Hb)、胆固醇(TC)、转铁蛋白(TF)及血浆瘦素浓度(Leptin)的变化。结果与治疗前比较,治疗组Scr、BUN和Leptin降低(P<0.01或P<0.05),Alb、Hb、TF和nPNA升高(P<0.01或P<0.05);对照组Scr、BUN和Leptin降低(P<0.01或P<0.05),Alb、TF和nPNA升高(P<0.01)。与对照组比较,治疗组Scr下降更明显(P<0.05)。结论排毒保肾丸能降低CAPD患者Scr、BUN和Leptin,升高Alb、Hb、TF和nPNA,具有改善营养状况的作用。Objective To observe the effect of Paidu Baoshen pill（modified Dhuang Zhechong pills） on nutrition of peritoneal dialysis.Methods 80 cases of peritoneal dialysis patients were ramdoly divided into treatment group of 40 cases and control group of 40 cases.On the basis of conventional therapy,the treatment group and control group were given Paidu Baoshen pill and compound keto acid tablets respectively for 6 months.Then the change of albumin（Alb）,creatinine（Scr）,urea nitrogen（BUN）,normalized protein equivalent of nitrogen appearance（nPNA）,hemoglobin（Hb）,cholesterol（TC）,tran TFerrin（TF） and plasma leptin（Leptin） were observed.Results Comparing with before treatment,the Scr,BUN and Leptin decreased（P〈0.01 or P〈0.05）,the Alb,Hb,TF and nPNA increased（P〈0.05 or P〈0.01） in the treatment group.The Scr,BUN and Leptin decreased（P〈0.01 or P〈0.05）,the Alb,TF and nPNA increased（P〈0.05 or P〈0.01） in the control group.Comparing with the control group,the Scr in the treatment group decreased（P〈0.05）.Conclusion Paidu Baoshen pill can reduce the Scr,BUN and Leptin,increase the Alb,Hb,TF and nPNA,and improve the nutrition.
