摘  要:目的比较单采血小板与手工分离血小板的质量及输注疗效,为临床合理应用两种血小板制品提供参考依据。方法随机抽查作者医院2015-01/2016-12月入库的100袋单采血小板和100袋400ml全血手工分离的浓缩血小板的血小板、红细胞和白细胞含量,比较两种血小板的质量。随机选取54例输注单采血小板患者和52例输注手工分离血小板患者,分别计算输注24h后的血小板校正增加指数(corrected count increment,CCI)和血小板回收率(percentage of platelet recovery,PPR),并统计输血不良反应的发生例数,判断其输注疗效。结果 1U单采血小板与12U手工分离血小板的血小板含量差异无统计学意义(P>0.05);单采血小板的白细胞含量、红细胞含量低于手工分离血小板,差异有统计学意义(P<0.05)。两组输注血小板患者的输注疗效、输血反应发生率的差异均无统计学意义(P>0.05)。血液系统疾病患者与非血液系统疾病患者输注血小板的疗效差异无统计学意义(P>0.05)。结论手工分离血小板具有一定的有效性和安全性,可作为单采血小板紧缺状况下有力的补充,帮助缓解当今血小板供应紧张的局面。Objective To compare the qualities and curative effects of single-donor plateletapheresis (SDP) and huffy-coat-collected platelets concentrates (BC-PC), and provide a reference for rational use of two platelet concentrates. Methods A total of 100 packs of SDP and 100 packs of 400 ml BC-PC from whole blood in authors department from Jan- uary 2015 to December 2016 were detected. The number of blood platelets, red blood ceils (RBC) and white blood cells (WBC) were assayed and compared for qualities. Fifty-four patients transfused with SDP and fifty-two patients transfused with BC-PC were randomly selected in author's hospital. The corrected count increment (CCI) and percent platelet recov- ery (PPR) after 24-hour infusion were calculated, and rate of evaluation adverse transfusion reactions as well. Results There were no significant differences in the content of platelets between BC-PC (12U) and SDP (1U) (P〉0. 05), but the white cell and erythrocytes assay in SDP were less than those in BC-PC (P〈0. 05). No differences were found in CCI, PPR and rate of adverse transfusion reaction between two groups (P〉0. 05). There were no differences in the curative effects of platelet infusion between hematopathic and nonhematopahic patients (P〉0.05). Conclusion BC-PC has certain effectiveness and safety. In order to alleviate the situation of platelet supply shortage, BC-PC can be used as powerful supplementary.
