早期联合治疗对陈旧性股骨干骨折手术治疗效果的影响

摘　　要：目的:对陈旧性股骨干骨折术前早期进行骨科医师、康复医师及护理的联合指导膝关节功能工作,观察早期引入功能锻炼治疗的的疗效。方法:将符合研究标准的股骨干骨折患者随机分为两组,一组为联合治疗组,入院后骨科医师与收治科室医师、康复师、护理进行联合治疗及时调整骨牵引、进行适当的早期膝关节功能锻炼。另一组为对照组,骨科医师进行骨牵引处理后收入需行优先急诊处理的普外、胸外、神经外科等科室,待病情平稳后转入骨科进行手术治疗。根据纳入标准,每组各收集病例30人。对两组的年龄、手术时机、手术时间、术中是否进行股骨的短缩处理、术后3个月膝关节屈膝功能予以记录并进行统计学分析。结果:两组患者的年龄、手术时机差异均无统计学意义。联合组手术时间及术中行股骨短缩例数少于对照组;联合组术后3个月膝关节主动屈曲活动范围优于对照组。结论:早期联合治疗,术前引入膝关节功能治疗、规范化牵引,更有利陈旧性股骨干骨折的手术治疗,有利于恢复更好的膝关节功能。Objective： To study the effect of orthopedic surgeon,rehabilitation physician and nursing joint on the function of knee joint in the early stage of femoral shaft fracture,and to observe the effect of early done of functional exercise therapy. Methods： Patients with femoral shaft fractures were randomly divided into two groups. One group was treated with combination therapy group,orthopedic surgeon and physician,rehabilitation group and nursing treatment. The bone traction was performed in time and early knee function exercise.The other group for the control group,orthopedic surgeon after traction treatment income should be prior to the emergency treatment of general,chest,neurosurgery and other departments,to be stable after the transfer into orthopedic surgery. According to the inclusion criteria,each group collected 30 cases. The age of the two groups,the timing of operation,the time of operation,the shortening of femur,and the knee knee flexion function were recorded and statistically analyzed. Results： There was no significantly difference in the age and operation time between the two groups. The operative time and the number of femoral shortening in the combined group were significantly lower than those in the control group. The active flexion range of the knee joint was better than that of the control group at 3 months after operation. Conclusion： Early combination therapy,preoperative knee functional therapy,standardized traction,more favorable surgical treatment of old femoral shaft fractures,is conducive to the recovery of better knee function.
