摘  要:目的:研究改良尿管拔除模式对脊柱术后患者拔除尿管后尿潴留及排尿舒适度的影响,从而寻求最佳的留置尿管拔除时机。方法:应用国际排尿障碍症状评分(IPSS)进行病例排除后,选取符合标准的脊柱手术留置尿管的住院患者360例,依据所属责任护士分管床位不同分为观察组和对照组,每组180例。观察组采用改良尿管拔除模式,即"放-放-冲"三步尿管拔除;对照组采用待患者膀胱排空后拔除尿管。比较两组患者拔管后首次自然排尿的等待时间、排尿舒适度及尿潴留的发生率。结果:与对照组比较,观察组拔管后首次自然排尿的等待时间短、排尿舒适度高、尿潴留的发生率低,差异具有统计学意义(P<0.01)。结论:采用改良尿管拔除模式可显著缩短患者首次自然排尿的等待时间、提高排尿舒适度,并降低尿潴留的发生率。Objective:To observe the effect of modified catheter extraction mode on urinary retention and voiding comfort after catheter removal in patients with spinal surgery,and to find the best time for catheter removal.Methods:After using international prostate symptom score(IPSS)for case exclusion,a total of 360 patients with indwelling catheters in the standard spine surgery were divided into observation group and control group according to the nurse in charge of beds,180 cases in each group.The observation group was treated with modified catheter extubation mode,the control group was treated with bladder emptying after catheter removal.The first natural urination waitting time after extubation,arination comfort and the incidence of urinary retention were compared.Results:The first natural urination waiting time after extubation,and the incidence of urinary retention in observation group were lower than those in control group,the urination comfort was higher than that in control group(P0.01).Conclusion:The modified urinary catheter extraction mode can significantly shorten the waiting time of patients for the first time,improve urination comfort,and reduce the incidence of urinary retention.
