复律宁颗粒联合西药治疗气阴两虚型慢性心力衰竭合并心律失常40例临床观察

摘　　要：目的观察复律宁颗粒治疗气阴两虚型心力衰竭合并心律失常的疗效和安全性。方法将80例气阴两虚型心力衰竭合并心律失常患者分为治疗组和对照组各40例,对照组予心力衰竭合并心律失常的西医规范化治疗,治疗组在西医规范化治疗基础上加用复律宁颗粒,两组均治疗4周。比较两组患者治疗前后心功能分级、心脏超声指标[包括射血分数(EF)、每搏输出量(SV)、左心室舒张末期内径(LVEDD)]、B型脑钠肽(BNP)、6分钟步行试验、中医证候积分、明尼苏达心力衰竭生活质量调查表(MLHFQ)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分,评价两组心功能疗效及心律失常疗效。结果治疗后两组心功能疗效(治疗组总有效率80.00%,对照组为55.00%)及心律失常疗效(治疗组总有效率70.00%,对照组为42.50%)比较治疗组优于对照组(P<0.05或P<0.01)。两组治疗后中医证候积分、6分钟步行距离、MLHFQ积分、HAMA积分、HAMD积分均较治疗前明显改善,且治疗组优于对照组(P<0.01)。两组治疗后BNP水平较治疗前均降低(P<0.01),且两组间比较差异无统计学意义(P>0.05)。两组治疗前后心脏超声各指标比较差异均无统计学意义(P>0.05)。结论复律宁颗粒治疗气阴两虚型心力衰竭合并心律失常患者能有效改善心功能,减少患者的早搏发作,改善患者的临床症状和生活质量。Objective To examine the effect and safety of Fulyuning Keli（ 复律宁颗粒） in treating heart failure complicated with arrhythmia of both Qi and Yin deficiency type. Methods Eighty heart failure complicated with arrhythmia patients of both Qi and Yin deficiency type were divided into treatment group and control group,40 cases in each group. The control group was given western medicine standardized treatment of heart failure complicated with arrhythmia. In addition to the standardized treatment,the treatment group was given Fulyuning Keli. Both groups were treated for 4 weeks. Patients＇ cardiac functional grading,cardiac ultrasound indexes including ejection fraction（ EF）,stroke volume（ SV） and left ventricular end diastolic dimension（ LVEDD）,brain natriuretic peptide（ BNP）,6-min walking test,Chinese medicine syndrome score,as well as the scores of Minnesota life quality of heart failure questionnaire（ MLHFQ）,Hamilton anxiety scale（ HAMA） and Hamilton depression scale（ HAMD） before and after treatment were compared. Heart function efficacy and arrhythmia efficacy in both groups were evaluated.Results After treatment,changes of heart function effect（ total effective rate 80. 00% in the treatment group,55. 00% in the control group） and arrhythmia effect（ total effective rate 70. 00% in the treatment group,42. 50% in the control group） was significantly different（ P 〈0. 05 or P 〈0. 01）. After treatment,Chinese medicine syndrome score,6-min walking distance,as well as the scores of MLHFQ,HAMA and HAMD in both groups improved,and the treatment group was superior to the control group（ P 〈0. 01）. BNP level in both groups decreased after treatment（ P〈 0. 01）. The difference between both groups was not significant（ P 〉0. 05）. The difference of cardiac ultrasound indexes in both groups before and after treatment was not significant（ P〉 0. 05）.
