摘  要:目的探讨64排宝石能谱CT追踪冻结技术(SSF)及不同期相重建在中等心率(65~75次/min)冠状动脉成像中对改善冠状动脉图像质量的作用。方法选取2015年5月至2016年1月来赤峰市医院行冠状动脉CT血管成像的患者,心率为65~75次/min的240例患者为研究对象,采用回顾性心电门控扫描模式分别进行标准算法重建、75%期相SSF算法重建、45%期相SSF算法重建。由两名高年资主治医师双盲采用Likert5分制评分法对不同算法重建图像进行图像质量评价,记录患者有效剂量、图像质量评分等。结果冠状动脉分支水平75%期相图像质量评分SSF算法总体评分、前降支(LAD)、回旋支(LCX)、右冠状动脉(RCA)评分均高于STD算法重建评分[(3.82±1.07)分比(3.49±1.11)分,(3.78±0.89)分比(3.18±0.94)分,(3.80±0.83)分比(3.28±0.96)分,(3.55±0.86)分比(3.39±1.01)分](P<0.05)。冠状动脉成像45%期相重建在分支水平图像质量评分,总体评分、LAD、LCX、RCA SSF算法评分均高于STD算法评分[(3.81±1.07)分比(3.49±1.11)分,(3.56±0.84)分比(3.18±0.94)分,(3.77±0.76)分比(3.28±0.96)分,(3.73±0.90)分比(3.39±1.01)分](P<0.05)。LAD在75%期相SSF重建评分高于45%期相SSF重建图像评分[(3.78±0.89)分比(3.56±0.84)分](P<0.05);RCA在45%期相SSF重建图像评分高于75%期相SSF重建图像评分[(3.73±0.90)分比(3.55±0.86)分](P<0.05)。冠状动脉节段水平75%期相、45%期相图像质量总评分分别为(3.66±1.06)分、(3.66±1.30)分,均高于STD算法重建评分(3.38±0.99)分(P<0.05)。结论宝石能谱CT冠状动脉成像SSF算法重建可以明显提高冠状动脉图像质量。LAD SSF算法重建75%期相较45%期相图像质量好,RCA SSF算法重建45%期相较75%期相图像质量好,LCX SSF算法重建两种期相图像质量评分无差别。Objective To evaluate the role of 64-slice gem energy spectrum CT snapshot freeze(SSF) and phase recon struction technique in improving the image quality of coronary computed tomographic angiography (CCTA) of patients with middle heart rate ranging 65-75 beats per minute(bpm). Methods A total of 240 patients with heart rate from 65 bpm to 75 bpm who underwent CCTA in Chifeng Municipal Hospital from May 2015 to Jan. 2016 were enrolled. The standard recon struetion algorithm(STD) ,75% R-R interval phase SSF reconstruction and 45% R-R interval phase SSF reconstruction were reconstructed with retrospective ECG-gating scanning mode. Likert 5-points score, which were interpreted by two expe- rienced radiologists according to the the double-blind method, was used as the image quality criterion. The effective dose (ED), image quality score of the patients were recorded. Results 75% SSF reconstruction image quality showed higher interpretability than standard reconstruction image quality score on coronary per-artery leveltotal per-artery level, anterior descending branch ( LAD ), circumflex branch ( LCX ), and right coronary artery ( RCA ) [ ( 3.82 ±1.07 ) vs ( 3.49 ± 1.11),(3.78±0.89) vs (3.18 ±0.94),(3.80±0.83) vs (3.28 ±0.96),(3.55 ±0.86) vs (3.39±1.01) ] (P 〈 0. 05 ). 45% SSF reconstruction image quality showed higher interpretability than standard reconstruction image quality score on coronary per-artery level, total per-artery level, LAD, LCX, RCA as well [ (3.81± 1.07 ) vs (3.49 ±1.11 ), (3.56 ±0.84) vs (3.18 ±0.94) ,(3.77 ±0.76) vs (3.28±0.96) ,(3.73 ±0.90) vs (3.39 ±1.01)1 (P 〈0.05).75%SSFreconstruction showed higher interpretability than 45% SSF on LAD[ (3.78 ±0.89) vs (3.56 ±0.84) ] (P 〈0.05) ;45% SSF reconstruction showed higher interpretability than 75% SSF on RCA[ (3.73 ±0.90) vs (3.55 ±0.86) ] (P 〈0.05). 75% and 45% R-R interval phase SSF reconstruction
