耳穴压豆辅助止痛药物治疗肾绞痛的疗效研究

摘　　要：目的探讨耳穴压豆辅助止痛药物治疗肾绞痛的疗效;方法收集住院行保守治疗≥3天的上尿路结石患者共60例,随机分成常规药物止痛组和耳穴压豆辅助止痛组,前者单纯予吲哚美辛栓塞肛和盐酸哌替啶针肌注止痛,而后者在上述常规止痛治疗的基础上,同时予以耳穴压豆治疗;观察两组患者治疗期间肾绞痛情况、药物使用情况。结果辅助耳穴压豆治疗第1天,两组患者间肾绞痛发作次数、肾绞痛VAS评分、吲哚美辛栓及盐酸哌替啶的用量差异均无统计学意义(P>0.05);而辅助耳穴压豆治疗第二、第三天,辅助耳穴压豆止痛组的每天疼痛发作次数、疼痛VAS评分、吲哚美辛栓及盐酸哌替啶用量均低于常规药物止痛组,差异有统计学意义(P<0.05)。结论针对肾绞痛患者,在常规予以解痉止痛药物快速、强烈止痛的基础上,辅以耳穴压豆中医治疗,能显著降低患者后续肾绞痛发作的频率和疼痛程度,同时也可减少止痛药物的用量,从而减少止痛药物可能带来的副作用。Objective To evaluate the effect of ‘ear acupoint bean pressing therapy ＇ in assistingthe conventional drug therapy in the treatment of renal colic. Method 60 patients having been treated with conventional therapy for upper urinary tract calculi with renal colic for over three days were firstrandomly divided into a control and an experimental group. The control group were treated with conventionalanalgesic drugs together with anal application of indometacin suppositories and intramuscular injection with meperidine hydrochloride. The experimental group wereadjuvantly treated with traditional Chinese medicine of ear acupoint bean pressing therapy in addition to that given to the control. The incidence of renal colic and drug application of both groups were then observed and compared. Findings In the first day of ear acupoint bean pressing therapy,no significant differences were found between the groups in the times of incidence of colic,severity of colic and the amount of application of doses of indometacin and meperidine（ P〉0. 05）. However,in the second and third day of ear acupoint bean pressing therapy,the times of incidence of colic,the severity of colic and the amount of application of doses of indometacin and meperidine in the control groupwere significantly higher and severer than those in the experimental group（ P〈0. 05）. Conclusion For the patients with renal colic,on the basis of the routine analgesic medication for rapidly and intensely stopping pain,the adjuvant traditional Chinese medicine of ear acupuncture point bean pressing therapy can assist in reducing the frequency and severity of subsequently outbreaks of renal colic and amount of analgesic drugs and hence reduce the complications caused by the analgesic drugs and benefit the patients.
