摘　　要：目的探讨乳腺超区段切除联合高负压引流术治疗浆细胞性乳腺炎的疗效。方法选取57例浆细胞性乳腺炎患者，术前均行X线及超声检查，根据术前定位行乳腺超区段切除＋高负压引流术，分析患者治疗效果、术中体会及预后情况。结果57例浆细胞性乳腺炎患者一期愈合50例；复发1例，再次行扩大切除＋高负压引流术后痊愈；切口感染4例，愈合不佳，行对症换药、引流、理疗等方法处理后康复；乳腺变形、乳头歪斜2例，征求患者意见后未予特殊处理。术后随访6～14个月，患者肿块完全消失，乳头、乳晕及外观基本恢复较好，手术效果满意。结论浆细胞性乳腺炎具有发病突然、发展快、病情复杂、容易复发等特点，行乳腺超区段切除联合高负压引流术，可以有效治疗疾病，减少复发，是一种有效的治疗手段。Objective To investigate the effect of large segment mastectomy combined with high negative pressure drainage on the treatment of plasma cell mastitis. Methods Fifty-seven plasma cell mastitis patients were examined with X-ray and ultrasound before operation, and were treated by large segment mastectomy combined with high negative pressure drainage according to the pre-localized resection area. The treatment effect, intraoperative experience and prognosis were summarized and analyzed. Results Among the 57 cases of plasma cell mastitis, 50 cases healed at first intention ; one case suffered recurrence and recovered after treated by enlarged resection combined with high negative pressure drainage; four cases suffered incision infection with poor healing and recovered after symptomatic treatment, drainage and physiotherapy; two cases presented breast deformation, nipple skew and accepted no treat- ment with the patients＇ consent. After surgery, the patients were followed-up for 6 to 14 months. The mastitis mass completely disappeared, and the nipple and areola recovered well. The operation effects were satisfactory. Conclusions Plasma cell mastitis is characterized by acute onset, rapid progression, complicated condition and easy to relapse. Large segment mastectomy combined with high negative pressure drainage is an effective treatment which can effectively treat the disease and reduce recurrence.
