Ligasure痔切除术与传统痔切除术在Ⅲ～Ⅳ度混合痔治疗中的疗效对比

摘　　要：目的比较Ligasure痔切除术与传统痔切除术治疗Ⅲ~Ⅳ度混合痔的应用效果。方法选择2014年1月至2015年6月于北京市二龙路医院收治的120例Ⅲ~Ⅳ度混合痔患者为研究对象,随机分为治疗组(行ligasure痔切除术,n=60)和对照组(行传统痔切除术,n=60)。比较两组手术时间、术中出血量、住院时间,术后疼痛、出血、切口感染以及术后尿潴留情况。结果两组手术时间差异无统计学意义(P>0.05)。治疗组术中出血量、住院时间均显著少于对照组,差异均有统计学意义(均P<0.05)。治疗组术后12 h、术后第1、3、7 d疼痛评分均显著低于对照组,差异均有统计学意义(均P<0.05)。两组术后均无术后出血及切口感染病例。治疗组术后发生尿潴留5例,对照组发生尿潴留6例,差异无统计学意义(P>0.05)。结论与传统痔切除术相比,Ligasure痔切除术治疗Ⅲ~Ⅳ度混合痔更利于减少术中出血量、缩短住院时间。Objective To compare the clinical effects of Ligasure hemorrhoidectomy and traditional hemorrhoidectomy in the treatment of mixed hemorrhoids of grade 3-4 degrees. Methods 120 cases of patients with stage Ⅲ and Ⅳ from January 2014 to June 2015 in Beijing Erlonglu hospital were selected as our subjects and randomly divided into treatment group（Liga Sure hemorrhoidectomy, n =60） and control group（conventional hemorrhoidectomy, n = 60）. The operation time, blood loss, hospitalization time, postoperative pain, bleeding, incision infection and postoperative urinary retention were compared between the two groups. Results There was no significant difference in the operation time between the two groups（P 0.05）. The amount of bleeding and the length of stay in the treatment group were significantly less than those in the control group（P 0.05）. The pain scores of the treatment group after 12 h and first, third, seventh days after operation were significantly lower than those of the control group（P 0.05）. And there were no postoperative bleeding and incision infection in the two groups. There were 5 cases of urinary retention in the treatment group and 6cases of urinary retention in the control group, the difference was not statistically significant（P 0.05）. Conclusion Compared with traditional hemorrhoidectomy, Ligasure hemorrhoidectomy is more effective in reducing the amount of bleeding and shortening the length of hospital stay.
