摘 要:目的探讨2型糖尿病合并下肢血管病变与机体DNA氧化损伤的关系。方法纳入门诊85例糖化血红蛋白<9%的2型糖尿病病人作为研究组,按照踝肱比(ABI)水平将上述病人分为糖尿病合并下肢血管病变组(n=35)及无并发症组(n=50);同时取60例正常人群作为正常对照组。使用ELISA方法测定血液单核细胞DNA氧化损伤指标8羟基脱氧鸟嘌呤(8-OHdG)水平,并在3组间进行比较,同时采用Logistic回归分析糖尿病合并下肢血管病变的危险因素。结果糖尿病合并下肢血管病变组与无并发症组间年龄、病程、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白等指标差异无统计学意义(P>0.05)。糖尿病合并下肢血管病变组血液8-OHd G水平为(4.03±1.89)ng/ml,无并发症组血液8-OHd G水平为(2.74±1.64)ng/ml,均较正常对照组[(0.53±0.53)ng/ml]升高(P<0.05),且糖尿病合并下肢血管病变组较无并发症组亦升高(P<0.05)。Logistic回归分析显示,8-OHd G以及LDL-C是糖尿病合并下肢血管病变的危险因素。结论 2型糖尿病合并下肢血管病变病人体内DNA氧化损伤较正常人及无并发症糖尿病病人严重,提示DNA氧化损伤在糖尿病下肢血管病变中发挥了一定程度的致病作用。Objective To investigate the relationship between diabetic peripheral vascular disease and oxidative damage to DNA in patients with type 2 diabetes mellitus( T2DM). Methods Patients with diabetes mellitus were divided into peripheral vascular disease group( n = 35) and non-complication group( n = 50). At the same time,60 healthy subjects were enrolled as normal control group. DNA was extracted from mononuclear cells from whole blood,and the level of serum 8-hydroxy-2'-deoxyguanosine( 8-OHd G) was detected and compared between the three groups. Logistic regression analysis was used to identify the relationship between 8-OHd G and diabetic peripheral vascular disease. Results T2DM patients had significantly higher level of 8-OHd G than control group. In addition,the level of 8-OHd G in peripheral vascular disease group was significantly higher than that of the patients without complication( P〈0. 05)). Logistic regression showed that 8-OHd G and low density lipoprotein cholesterol were risk factors of diabetic peripheral vascular disease. Conclusions T2DM patients with diabetic peripheral vascular disease showed greater oxidative damage to DNA,which is a risk factor of diabetic peripheral vascular disease.
