N端B型利钠肽预测老年慢性房颤病人左心房血栓的价值

摘　　要：目的探讨N端B型利钠肽(NT-pro BNP)在评估老年慢性房颤病人左心房血栓形成中的临床价值。方法对78例老年慢性房颤病人实施经食道超声检查,根据有无左心房血栓分为左心房血栓组和无左心房血栓组,检测并比较2组病人的NT-pro BNP水平及其临床资料。采用Logistic回归分析影响病人左心房血栓形成的因素。结果左心房血栓组的NT-pro BNP水平为(299.60±20.09)pg/ml,显著高于无左心房血栓组的(174.19±18.16)pg/ml,差异有统计学意义(P<0.05)。多因素回归分析表明,高NT-pro BNP水平是老年慢性房颤病人发生左心房血栓的独立预测指标(OR=1.11,95%CI:1.01~1.16,P=0.03)。结论 NT-pro BNP水平可作为预测老年慢性房颤病人左心房血栓的指标。Objective To explore the predictive value of N-teminal pro B-type natriuretic peptide（ NT-pro BNP） for left atrial thrombosis in elderly patients with chronic atrial fibrillation.Methods A total of 78 patients with chronic atrial fibrillation were divided into two groups according to left atrial thrombosis by transesophageal echocardiography（ TEE）. P lasma levels of NT-pro BNP and other clinical data were recorded and analyzed. Results The level of NT-pro BNP was significantly higher in the patients with left atrial thrombus（ AT group） than that in the patients without left atrial thrombus（ LAT group） [（ 299. 60 ± 20. 09 pg/ml） vs.（ 174. 19 ± 18. 16 pg/ml）,P〈0. 05]. Multivariate Logistic regression analysis demonstrated that NT-pro BNP level was an independent predictor for thrombus of the left atrium（ OR = 1. 11,95 % CI： 1. 01-1. 16,P = 0. 03）.Conclusions NT-pro BNP level predicts independently left atrial thrombosis in elderly patients with chronic atrial fibrillation.
