应用涤纶心脏修补材料联合皮瓣移植修复下睑全层缺损的临床研究

摘　　要：目的:探索下睑全层缺损修复新方法。方法:笔者科室2014年6月至2016年6月共收治下睑全层缺损患者9例,应用涤纶心脏修补材料联合下睑局部皮瓣移植再造下睑5例,联合带蒂额部岛状皮瓣移植再造3例,单纯下睑皮瓣移植再造1例。结果:术后随访6~24个月,6例患者下睑再造成功,术后睑裂闭合良好,外观得到明显改善,无感染及排斥反应。3例因涤纶心脏修补材料脱出失败;1例单纯下睑局部皮瓣移植再造下睑后下睑外翻,再次手术置入涤纶心脏修补材料下睑外翻矫正,外形良好。结论:应用涤纶心脏修补材料联合皮瓣移植修复下睑全层缺损疗效确切,但也有涤纶心脏修补材料排斥脱出风险。Objective To explore a new method for repair of full-thickness defect of lower eyelid. Methods We reconstructed lower eyelids in 9 cases from June 2014 to June 2016, one case by local skin fap transplantation, 5 cases by heart Dacron graft combined with local skin fap transplantation, and 3 cases by heart Dacron graft with forehead island skin fap transplantation. Results Followed up for 6-24 months, the reconstruction succeeded in 6 cases without infection or rejection, patients achieving good appearance of lower eyelids. 3 cases failed due to heart Dacron graft detachment. One case with local skin fap transplantation, the patient experienced lower eyelid ectropion, but it was corrected in a second surgery with heart Dacron graft. Conclusion Heart Dacron graft transplantation combined with skin fap is effective in correcting full-thickness inferior blepharocoloboma. However, there is a risk of graft detachment due to rejection.
