慢性咳嗽肺阳虚证辨治探讨

摘　　要：慢性咳嗽肺阳虚证临床常见,历代医家早有认识,从对肺阳和肺阳虚的认识、肺阳虚咳嗽的病因病机、临床表现、治法方药以及典型病案等方面进行了探讨。慢性咳嗽肺阳虚证的病因主要有外邪伤肺、饮食失节、过用寒凉药品、脾肾等他脏虚损所累以及禀赋肺阳不足等;病机为肺阳亏虚兼风邪伏肺致肺失宣降而咳;临床表现以后背寒冷症状尤为突出,"背寒冷如掌大"或可视为慢性咳嗽肺阳虚证的主症之一与辨证要点;治以疏风宣肺、温阳健脾,方用苓桂术甘汤合止嗽散加减,临床取得较好疗效。Yang deficient syndrome of chronic cough is very common in clinic. Doctors of all dynasties have described this,and they explores the recognition of lung yang,yang deficiency of lung,the etiology and pathogenesis,symptoms,treatment principles,prescriptions,and typical cases of yang deficient cough. The etiology of chronic cough yang deficiency syndrome are mostly external exogenous injures lungs,disorder of dietary,excessive use of cold medicine,asthenia-impairment of spleen and other visceral organs,as well as congenital insufficiency of yang in lung,et al. The pathogenesis yang deficiency of lungs and pathogenic wind hidden in lungs,which induces lungs could not dispersing and descending and cough happens as a result. The clinical manifestation-cold back is prominent. The therapeutic method should be dispelling wind for dispersing,warming yang for strengthening spleen,and modified Linggui Zhugan Decoction combined Zhisou Powder has good effect.
