摘  要:目的观察支气管哮喘患者外周血嗜酸性粒细胞(EOS)的计数,及其与呼出气一氧化氮(FeNO)和诱导痰EOS百分比的相关性,从而寻找与哮喘相关的炎症因子,预防哮喘急性发作。方法收集2015-09~2016-12期间首都医科大学宣武医院哮喘患者100例,其中急性发作期72例,非急性发作期28例,收集30例健康志愿者作为对照组。检测各组外周血EOS计数和诱导痰EOS百分比,测定呼出气FeNO水平和FEV1%水平。结果哮喘急性发作组与非急性发作组外周血EOS、呼出气FeNO、诱导痰EOS百分比均高于健康对照组,差异有统计学意义(P<0.05)。哮喘急性发作组外周血EOS计数与呼出气FeNO水平和诱导痰EOS百分比均呈现正相关(r=0.392,P<0.05;r=0.321,P<0.05);非急性发作组外周血EOS计数与呼出气FeNO水平和诱导痰EOS百分比均呈现正相关(r=0.435,P<0.05;r=0.439,P<0.05)。而对照组外周血EOS计数与呼出气FeNO水平和诱导痰EOS比例无相关性(r=-0.357,P>0.05;r=0.170,P>0.05)。结论哮喘患者外周血EOS计数增高,并且与呼出气FeNO、诱导痰EOS比例呈现正相关。外周血EOS水平在哮喘急性发作期明显高于非急性发作期,其在一定程度上能够反映哮喘患者气道炎症水平,并预测哮喘的急性发作。Objective To investigate the eosinophils( EOS) count in peripheral blood and its relationship with exhaled nitric oxide(Fe NO) and percentage of induced sputum EOS in asthmatic patients for looking for the asthma-related inflammation factors to prevent the acute asthma exacerbation. Methods One hundred patients with bronchial asthma in Xuanwu Hospital of Beijing Capital Medical University from September 2015 to December 2016 were enrolled,including 72 patients with acute exacerbation of bronchial asthma(acute exacerbation group) and 28 patients with non-acute exacerbation of bronchial asthma(non-acute exacerbation group),respectively. Thirty healthy volunteers were chosen as control group. The levels of Fe NO and FEV1% were measured by Fe NO analyzer and pulmonary function. The EOS count in peripheral blood and percentage of induced sputum EOS were measured. Results The EOS count in peripheral blood,the exhaled nitric oxide,and the percentage of induced sputum EOS in acute exacerbation group and non-acute exacerbation group were both higher than that in control group( P 〈 0. 05). The EOS count in peripheral blood was positively correlated with exhaled nitric oxide level and the percentage of induced sputum EOS in acute exacerbation group( r = 0. 392,P 〈 0. 05;r =0. 321,P 〈 0. 05). The EOS count in peripheral blood was positively correlated with exhaled nitric oxide level and the percentage of induced sputum EOS in non-acute exacerbation group( r = 0. 435,P 〈 0. 05;r = 0. 439,P 〈 0. 05). The EOS count in peripheral blood was not correlated with exhaled nitric oxide level and the percentage of induced sputum EOS in control group( r =-0. 357,P 〉 0. 05;r= 0. 170,P 〉 0. 05). Conclusion The EOS count in peripheral blood increases in asthma patients and is positively correlated with exhaled nitric oxide and EOS in induced sputum. EOS count in acute exacerbation bronchial asthma is significantly higher than that of non-acute exacerbation. Those can reflect the level of airway
