老年腰椎滑脱经皮椎弓根钉内固定联合椎体间融合术治疗的效果探讨

摘　　要：目的本次研究主要分析对老年腰椎滑脱患者采用经皮椎弓根钉内固定联合椎体间融合术治疗的临床治疗效果。方法 60例老年腰椎滑脱患者作为研究对象,采用随机盲选法将其分为对照组与观察组,各30例。对照组患者采用常规开放椎体间融合术治疗,观察组患者采用经皮椎弓根钉内固定联合椎体间融合术治疗。将两组患者术中出血量、手术时间、术后活动时间、术后感染率、植骨融合率、止痛药使用率等指标作为对比依据。结果观察组术中出血量、手术时间、术后活动时间均明显优于对照组,差异均具有统计学意义(P<0.05)。观察组术后感染率、植骨融合率、止痛药使用率分别为3.3%、93.3%、6.7%,均明显优于对照组的20.0%、50.0%、26.7%,差异均具有统计学意义(P<0.05)。结论对老年腰椎滑脱患者采用经皮椎弓根钉内固定联合椎体间融合术治疗,有利于提高治疗效果,减少手术创伤,改善患者预后,可作为临床首选治疗方法推广。Objective To analyze the clinical treatment effect of percutaneous pedicle screw internal fixation combined with interbody fusion in the treatment of senile patients with lumbar spondylolisthesis. Methods A total of 60 senile patients with lumbar spondylolisthesis as study subjects were divided by random blind selection into control group and observation group, with 30 cases in each group. The control group was treated with conventional open interbody fusion, and the observation group was treated with percutaneous pedicle screw internal fixation and interbody fusion. Comparison were made on intraoperative bleeding volume, operation time, postoperative activity time, postoperative infection rate, bone graft fusion rate, analgesic drug use rate between two groups. Results The observation group had obviously better intraoperative bleeding volume, operation time and postoperative activity time than the control group, and the difference was statistically significant（P〈0.05）. The observation group had postoperative infection rate, bone graft fusion rate and analgesic drug use rate respectively as 3.3%, 93.3% and 6.7%, which were all obviously better than 20.0%, 50.0% and 26.7% in the control group, and their difference was statistically significant（P〈0.05）. Conclusion For senile patients with lumbar spondylolisthesis, combination of percutaneous pedicle screw internal fixation and interbody fusion treatment can improve the therapeutic effect, reduce surgical trauma and improve prognosis of patients. So it can be promoted as the preferred treatment method in clinic.
