摘　　要：目的 :探讨地佐辛术后镇痛对妇科恶性肿瘤患者血浆儿茶酚胺及免疫功能的影响。方法 :抽取本院近年收治的择期行妇科恶性肿瘤手术患者50例,按单、双号平均分为对照组和观察组各25例,给予观察组患者地佐辛与托烷司琼静脉滴注实施术后镇痛、对照组组患者芬太尼与托烷司琼静脉滴注实施术后镇痛,对两组患者镇痛情况及不良反应进行分析比较。结果 :观察组患者术后2h、6h及24h的安静和活动状态VAS疼痛评分显著低于对照组(P<0.05);两组E、NE、DOP有明显差异(P<0.05);观察组术后48 h的CD3+、CD4+及NK细胞活性较对照组显著升高(P<0.05)。观察组不良反应发生率显著低于对照组(P<0.05)。结论 :在妇科恶性肿瘤术后采用地佐辛镇痛效果较好,能有效对术后应激反应进行抑制,促进细胞免疫功能恢复。Objective：The effect of postoperative analgesia with dezocine on plasma catecholamine level and immune function in patients with gynecological malignant tumor.Methods：Fifty cases of patients with gynecologic malignant tumor underwent surgery in our hospital in recent years were selected and randomly divided into a control group and an observation group with 25 cases each.The postoperative analgesia was carried out with dezocine and tropisetron in the observation group and fentanyl and tropisetron in the control group by intravenous infusion and analgesic condition and adverse reactions were analyzed and compared between the two groups.Results：VAS scores of quiet and active state at 2h,6 h and 24 h of postoperation were significantly lower in the observation group than in the control group（P〈0.05）.There were significant differences in E,NE and DOP between the two groups（P〈0.05）.The CD3＋ and CD4＋ levels and NK cell activity at 48 h of postoperation were significantly higher while the incidence of adverse reactions was significantly lower in the observation group than in the control group（P〈0.05）.Conclusion：The effectiveness of postoperative analgesia in the gynecological malignant tumor is much better when dezocine is used,which can effectively inhibit the postoperative stress reaction and promote the recovery of cellular immune function.
