摘　　要：目的探讨社区获得性肺炎(CAP)患者外周血内淋巴细胞计数(LY)水平与CD_3^+、CD_4^+、CD_8^+T淋巴细胞水平的相关性。方法回顾性分析本院2013年7月至2014年12月收治的200例CAP患者的临床资料,根据肺炎CURB-65评分将患者进行分组,其中低风险组(评分1~2分,100例),高风险组(评分3~5分,100例),同时纳入100名健康对照者。比较3组患者外周血内LY水平与CD_3^+、CD_4^+、CD_8^+T淋巴细胞水平的差异,并评价各指标之间的相关性。结果 CAP患者LY水平及CD_3^+、CD_4^+、CD_8^+T淋巴细胞水平均明显低于对照组,差异有统计学意义(P<0.05);高风险组LY水平及CD_3^+、CD_4^+、CD_8^+T淋巴细胞水平均明显低于低风险组,差异有统计学意义(P<0.05);LY水平与CD_3^+、CD_4^+、CD_8^+T淋巴细胞水平成显著正相关(P<0.05)。结论外周血内淋巴细胞计数水平可用于社区获得性肺炎患者细胞免疫功能的评估。Objective To investigate the CD3＋,CD4＋,CD8＋ T lymphocyte counts in peripheral blood of patients with community-acquired pneumonia（CAP）. Methods The clinical data of 200 patients with community-acquired pneumonia admitted to our hospital from July 2013 to December 2014 were analyzed retrospectively. According to CURB-65 scores,total 200 patients with CAP were divided into low risk group（score 12,n=100）and high risk group（score 35,n=100）. At the same time,100 healthy controls were enrolled in the study. Lymphocyte counts of CD3＋,CD4＋,CD8＋ T lymphocyte were compared among the 3 groups and their correlation was analyzed. Results The peripheral blood lymphocyte counts of CD3＋,CD4＋,CD8＋ T lymphocyte in the CAP patients were significantly lower than those in the control group（P〈0.05）. The lymphocyte counts of CD3＋,CD4＋,CD8＋ T lymphocyte in the high risk group was lower than those in the low risk group（P〈0.05）. Conclusion CD3＋,CD4＋ and CD8＋ T lymphocyte counts were positively correlated with risk of community-acquired pneumonia.
