胰岛素及康惠尔渗液治疗重度压疮患者的临床效果比较

摘　　要：目的比较胰岛素混合液与康惠尔渗液对Ⅱ期以上压疮患者肉芽组织生长及创面血管化的影响。方法选取本院2014年2月至2016年2月100例被确诊患有Ⅱ期以上压疮的患者,按统计学随机数字表将患者随机分为胰岛素混合液组(52例)、康惠尔渗液组(48例),以创面愈合时间的长短等作为评价指标,应用统计学方法对实验结果进行分析。结果胰岛素混合液组、康惠尔渗液组Ⅲ期患者创面愈合时间比较差异无统计学意义(t=1.724,P>0.05)。胰岛素混合液组、康惠尔渗液组Ⅳ期患者创面愈合时间比较差异无统计学意义(t=1.939,P>0.05)。患者治疗两周后Ⅲ期压疮创面胰岛素混合液组治疗有效率为65.2%,康惠尔渗液组治疗有效率为68.4%,两组治疗有效率差异无统计学意义(χ~2=1.265,P<005);Ⅳ期压疮创面胰岛素混合液组治疗有效率为33.3%,康惠尔渗液组治疗有效率为40.0%,两组差异有统计学意义(χ~2=5.658,P<0.05)。各阶段胰岛素混合液组、康惠尔渗液组创面面积减小值、创面肉芽面积变化值比较差异无统计学意义(P均>0.05)。结论胰岛素混合液治疗Ⅱ期以上压疮患者效果显著,和现代新型敷料康惠尔渗液的治疗效果相近;但是胰岛素的使用成本相对于康惠尔渗液成本低廉,易被广大患者接受。Objective To compare the effect of insulin mixture and Kanghuier exudate on granulation tissue growth and vascularization of wound in patients with pressure ulcer Ⅱ. Methods In our hospital from February 2014 to February 2016,100 cases were diagnosed with stage II pressure ulcer patients. According to the statistics of random number table,patients were randomly divided into insulin mixture group（52 cases）,and Kanghuier drainage group（48 cases）. Wound healing time was used as the assessment index. Results There was no significant difference in the wound healing time between the mixed insulin group and the Kanghuier group of stage III patients（t=1.724,P〉0.05）and stage IV patients（t=1.939,P〉0.05）. After two weeks of treatment,the effective rate for the stage III patients of the insulin mixture group was 65.2% patients,the effective rate of the liquid infiltration Kanghuier group was 68.4%;there was no significant difference（χ~2=1.265,P=0.748）.The effective rate of stage IV pressure ulcers of the insulin mixture group was 33.3%,and Kanghuier infiltration group was 40%;there was significant difference between two groups（χ~2=5.658,P〈0.05）. There was no significant difference in the reduction of the wound area and the change of granulation area of the mixed insulin group and the Kanghuier group（P〉0.05）.Conclusion Insulin mixture treatment of stage II patients had more significant effect,and the modern new dressing Kanghuier leachate treatment effect wassimilar;but the cost of insulin treatment was lower than that of Kanghuier exudates.
