经皮椎间孔镜技术与椎板开窗联合人工半椎板成形术治疗腰椎间盘突出症的临床比较观察

摘　　要：目的:观察经皮椎间孔镜技术与椎板开窗联合人工半椎板成形术治疗腰椎间盘突出症的临床疗效。方法:回顾安徽中医药大学第一附属医院骨科2013年1月-2016年6月诊治的单节段腰椎间盘突出症40例,经皮椎间孔镜治疗20例(微创组),椎板开窗联合人工半椎板成形术治疗20例(开放组)。完善相关术前检查,术后均予以抗炎、止痛、消肿等基础治疗。两组术后进行腰、腿痛VSA评分、改良Macnab疗效评定、手术切口长度、手术时间、住院天数的组间比较,最后进行相关统计学分析。结果:术前、术后6个月以及末次随访微创组与开放组腰、腿痛VAS评分比较,差异无统计学意义(P>0.05)。微创组较开放组手术切口长度小、手术时间短、住院天数少,差异有统计学意义(P<0.05)。结论:与椎板开窗联合人工半椎板成形术治疗腰椎间盘突出症比较,椎间孔镜技术具有手术创伤小、住院时间短等优势。Objective：To observe the clinical effects of percutaneous intervertebral foramen technology and laminectomy fenestration combined with artificial hemi laminoplasty on lumbar disc protrusion. Methods：A total of 40 patients with singlesegment lumbar disc protrusion who underwent orthopedics treatment at the first affiliated hospital of Anhui university of Chinese medicine from January 2013 to June 2016 were randomly divided into minimally invasive group（percutaneous intervertebral foramen technology）and open group（laminectomy fenestration combined with artificial hemi-laminoplasty）.The related preoperative examinations were improved and the anti-inflammatory,then analgesic and detumescence treatments were given.Lumbar and leg pain VSA score,efficacy evaluation of Macnab,incision length,operative time,hospital stay of patients in two groups were recorded and compared. Results：There were no statistical significance in VAS score between minimally invasive group and open group six months after operation and last follow-up（P〈0.05）. There were statistically differences in the length of incision,operation time and hospital stay between the minimally invasive group and the open group（P0.05）. Conclusion：Intervertebral foramen lens technology may be accepted easily by patients.
