电针腧穴“解郁方”治疗卒中后抑郁70例疗效观察

摘　　要：目的:讨论电针腧穴"解郁方"对卒中后抑郁的临床治疗价值。方法:70例卒中后抑郁住院患者,按照随机数字表法随机分为2组,每组35例,针刺组给予针灸治疗,以腧穴"解郁方"百会、神门、太冲为主穴,配合膻中、合谷、内关、太溪、悬钟、三阴交,施补法,日1次;对照组给予盐酸帕罗西汀片,20 mg/d口服。两组患者均针对原发病给予常规治疗,并尽早进行康复训练。住院集中治疗4 w,统计各组患者治疗前后HAMD及ADL指数的变化以及NIHSS评分的变化。结果:治疗后,针刺组患者总有效率及ADL高于对照组(P<0.05);HAMD、NIHSS低于对照组(P<0.05)。结论:电针腧穴"解郁方"治疗卒中后抑郁效果显著,能够有效降低患者HAMD评分,改善抑郁状态,提高日常生活能力,改善神经功能缺损程度。Objective：To investigate the clinical value of electroacupuncture at acupoints ＂Jieyu formula＂ in the treatment of post-stroke depression（PSD）. Methods：70 PSD patients were randomly divided into the acupuncture group and control group with 35 cases in each group.Two groups were given the same routine treatment and early rehabilitation. In addition,the acupuncture group was acupunctured using Jieyu formula（Baihui,Shenmen,Taichong,Danzhong,Hegu,Neiguan,Taixi,Xuanzhong and Sanyinjiao acupoints）,supplement method,once a day,six times a week. The control group was given paroxetine hydrochloride tablets,20 mg/d. One course of treatment was 4 weeks. Changes of Hamilton Depression Rating Scale（HAMD）score,daily living ability rating scale（ADL） and neurological deficit score NIHSS of patients were recorded.Results：After treatment,the total effective rate of acupuncture group was significantly higher than that of control group（P〈0.05）.Compared with control group,HAMD score and NIHSS score were decreased（P〈0.05）,ADL score of patients of two groups was improved（P〈0.05）. Conclusion：Electroacupuncture acupoints ＂Jieyu formula＂ could improve depression and the ability of daily life and degree of neural function defect of post-stroke depression patients.
