女性乙型肝炎病毒携带者卵巢颗粒细胞中乙型肝炎病毒感染情况与临床检测结果分析

摘　　要：目的分析女性乙型肝炎病毒(HBV)携带者卵巢颗粒细胞中HBV感染情况与临床检测结果。方法选取自2014年1月至2016年1月于自贡市第三人民医院接受辅助生育治疗的40例女性患者作为研究对象,其中,20例患者为HBV携带者(携带组),20例患者为非HBV携带者(非携带组),分离患者卵泡液中的颗粒细胞并制成涂片。应用经地高辛标记的特异性HBV DNA探针,通过原位杂交对颗粒细泡涂片进行检测;应用碱性磷酸酶系统进行显色反应。结果携带组患者的颗粒细胞涂片显示,5例可见200个成簇的颗粒细胞,细胞核中检测出20个HBV DNA阳性细胞;6例患者可见220个片状、单层的颗粒细胞,细胞核中检测出8个HBV DNA阳性细胞;9例患者可见180个成簇的颗粒细胞,细胞核中检测出4个HBV DNA阳性细胞。非携带组患者的颗粒细胞涂片为阴性。显色10 min时,患者未出现阳性信号;显色20 min时,4例患者出现阳性信号;显色30 min时,3例患者背景着色较深,阳性信号不易辨认,忽略不计。因此,20 min为显色的最佳时间。结论女性HBV携带者卵巢颗粒细胞中具有HBV DNA,可通过生殖细胞向子代传播乙型肝炎。因此,应加强对HBV携带孕产妇的健康宣传教育,并采取有效措施进行阻断。Objective To analyze the infection status and clinical detection of hepatitis B virus（ HBV） in ovarian granulosa cells of female hepatitis B carriers. Methods A retrospective study was performed on 40 cases of female patients who were admitted and were given assisted fertility treatment from January 2014 to January 2016. There were 20 cases of HBV carriers（ carrying group） and 20 cases of non-HBV carriers（ non-carrying group）. The ovarian granulosa cells were extracted and made into smears. A specific digoxigenin labeled hepatitis B virus DNA probe,the fine particle smears were detected by in situ hybridization. The color reaction was detected by alkaline phosphatase system. Results The granular cells of carrying group showed that 5 cases of 200 clusters with 20 HBV DNA positive cells; 6 cases of 220 clusters with single layer of granular cells and with 8 HBV DNA positive cells; 9 cases of 180 clusters with 4 HBV DNA positive cells. The smear of granule cells was negative in non-carrying group. No case had positive signals at 10 minutes of color rendering; 4 cases had positive signals at 20 minutes; 3 cases had darker background color and the positive signal was not obvious to be recognized and ignored at 30 minutes. Therefore,the best time for color rendering was at 20 minutes. Conclusion The hepatitis B virus carries ovarian granulosa cells which containing HBV and DNA can transmit hepatitis B to offspring through reproductive cells.Therefore,the health promotion of maternal health in HBV should be strengthened,and effective measures should be taken to block it.
