大骨节病患者手部X线征像的研究

摘　　要：目的探讨成人大骨节病患者手部X线影像学的特征性改变。方法在黑龙江、吉林和四川省大骨节病历史病区筛选成人大骨节病病人,对其手部X线片各种改变征象进行统计分析。结果在调查的1645名成人中,共检出大骨节病患者299人,其手部异常X线改变征像的检出率分别为假性囊肿17.73%(53/299)、骨棘(骨嵴、骨刺)56.52%(169/299)、粗隆增生与骨赘19.06%(57/299)、游离骨块11.37%(34/299)、关节面缺损或破坏14.38%(43/299)、基底增宽与纺锤样改变57.86%(173/299)、喇叭口状改变4.68%(14/299)、塌陷修复相12.04%(36/299)、掌骨头膨大(半脱位与镰状影)47.49%(142/299)和腕骨拥挤与变形46.49%(139/299)。结论基底增宽是成人大骨节病患者手部检出率最高的X线征像。Objective To investigate the hand X-ray image characteristics of Kaschin-Beck Disease（KBD） adult patients. Meth- od In the KBD historical endemic areas of Heilongjiang, Jilin and Sichuan province, statistical analysis of various signs of chan- ges in their hand X-ray images. Results 299 of 1 645 adults were diagnosed as KBD in the survey. The detection rate of abnor- mal X-ray changes was Pseudoeyst 17.73% （53/299）, Bone thorn in the edge of interphalangeal Joint （ Bone cristae Bone spur） 56.52% （169/299）, Hyperplasia of tuberosity and Osteophyte 19.06% （57/299）, Free bone fragments 11.37% （34/299）, Ar- ticular surface defects or destruction 14.38% （43/299）, Basal broadening or Spindle-shaped of the proximal end of phalanx 57. 86% （173/299）, Horn mouth shape change of proximal end of phalanx 4.68% （14/299）, Collapse repair phase of phalanx 12. 04% （36/299）, Swelling and semilnxation or Sickle shadow of metacarpal head 47.49% （142/299）, and Deformation and conges- tion of carpus 46.49% （139/299）, respectively. Conclusion Basal broadening is the hand X-ray image with the highest detec- tion rate in the adult KBD patients.
