摘  要:目的血清铁调节素(hepcidin)、生长分化因子15(GDF15)、促红细胞生成素(Epo)、转铁蛋白受体(sTfR)与cOPD伴发贫血的关系及临床应用价值。方法2014年1月至2016年12月选取本院呼吸内科收治的120例COPD患者,其中COPD合并贫血48例,COPD非贫血72例,另选取本院体检中心收治的40名健康体检者为对照组,应用酶联免疫吸附测定(EILSA)法测定各组血清hepcidin、GDF15、Epo、sTfR水平,并应用受试者特异性曲线(ROC)分析血清hepcidin、GDF15、Epo、sTfR在COPD伴发贫血中的应用价值。结果COPD合并贫血组、COPD非贫血组血清hepcidin、GDF15、Epo、sTfR水平均高于对照组(P〈0.05),而COPD合并贫血组血清hepcidin、GDF15、Epo、sTfR水平高于COPD非贫血组(P〈O.05)。COPD贫血组中重度贫血患者血清hepcidin、GDF15、Epo、sTfR水平显著高于轻中度贫血组(P〈0.05)。经ROC曲线分析可知,血清hepcidin、GDF15、Epo、sTfR在COPD并发贫血中诊断敏感度分别为82.2%、82.2%、81.8%、78.9%;特异度分别为89.8%、89.2%、80.7%、81.2%。结论血清hepcidin、GDF15、Epo、sTfR水平变化与COPD并发贫血有密切关系,可作为COPD并发贫血的诊断指标。Objective To investigate the relationship and the clinical application value between serum hepcidin, growth factor 15 (GDF15), erythropoietin (Epo), transferrin receptor (sTfR) and chronic obstructive pulmonary disease (COPD) associated with anemia. Methods 120 cases of patients with COPD were enrolled in Department of Respiratory Medicine from January 2014 to December 2016, including 48 patients with COPD with anemia and 72 patients with COPD, and 40 cases of healthy subjects were selected with control group. The levels of serum hepcidin, GDF15, Epo and sTfR were measured by enzyme linked immunosorbent assay (EILSA) method. The serum specific values of hepcidin, GDF15, Epo and sTfR in COPD were analyzed by subject specificity curve (ROC). Results The levels of serum hepcidin, GDF15, Epo and sTfR in COPD group were higher than control group ( P %0.05), but the level of serum hepcidin,GDF15,Epo and sTfR in COPD group was higher than COPD group ( P %0.05). The levels of serum hepcidin, GDF15, Epo and sTfR in patients with moderate to severe anemia were higher than mild to moderate anemia group ( P〈0.05). The diagnostic sensitivity of serum hepcidin, GDF15, Epo and sTfR in COPD complicated with anemia were 82.2%, 82.2% and 78.9% respectively. The specificity was 89.8%,89.2%,80.7% and 81.2%.Conclusions The changes of serum hepcidin,GDF15, Epo and sTfR are closely related to COPD complicated with anemia, and can be used as a diagnostic indexfor COPD complicated with anemia.
