摘 要:目的探讨益肾降浊冲剂治疗慢性肾脏病(Chronic kidney disease,CKD)3-5期患者前后Klotho蛋白及1,25(OH)_2D_3水平的影响。方法 76例CKD3、4、5期患者,30例正常对照组,以ELISA方法检测血清1,25(OH)_2D_3、Klotho水平,钙(Ca)、磷(P)、尿素氮(BUN)、血肌酐(SCr)由Beckman C800全自动生化分析仪测定;根据血钙磷浓度计算钙磷乘积(Ca×P);根据血尿Cr浓度和24h尿量计算内生肌酐清除率(CCr)。结果治疗前CKD3-5组Klotho、1,25(OH)_2D_3、Ca整体水平低于正常对照组,CKD3-5组P水平高于正常对照组,CKD3-5期治疗后Scr、BUN、P水平较治疗前下降,治疗后CKD3-5组Klotho、1,25(OH)_2D_3水平较治疗前无明显升高。结论益肾降浊冲剂短期内不能改善CKD3-5期患者血清klotho蛋白及1,25(OH)_2D_3水平,但可以改善其肾功能,降低血肌酐、尿素氮水平,改善钙磷代谢。Objective To investigate the effect ofYi Shen Jiang zhuo Granule on Klotho protein and 1,25 (OH) 2D3 levels in patients with chronic kidney disease (Chronic kidney disease, CKD) before and after 3-5 period. Method 76 cases of CKD3-5 patients, 30 cases of normal control group, using ELISA method for detection of serum 1,25 (OH) 2D3, the level of Klotho, calcium (Ca), phosphorus (P), urea nitrogen (BUN), serum creatinine (SCr) were measured by BeckmanC800 automatic biochemical analyzer; calculation of calcium phosphorus product according to the blood calcium and phosphorus concentrations (CaxP); according to the calculation of hematuria Cr concentration and 24h urine creatinine clearance rate (CCr). Result The level of Klotho, 1,25 (OH) 2D3, and Ca was lower in CKD3- 5 patients than the normal control group before treatment; The level of P in CKD3-5 group was higher than that in normal control group;The levels of Scr, BUN and P were decreased after treatment with CKD3-5;After treatment, the levels of Klotho, 1,25 (OH) 2D3 in CKD3-5 group were not significantly higher than those before treatment. Conclusion In the short term, the level of serum Klotho protein and 1,25 (OH)2D3 in patients with stage CKD3-5 can not be improved, but it can improve the renal function, decrease the level of serum creatinine, urea nitrot, en and imorove the metabolism of calcium and nhosDhorus.
