温肾活血法改善不孕症患者子宫内膜容受性的前瞻性队列研究

摘　　要：目的:评价温肾活血法改善不孕症患者子宫内膜容受性的疗效。方法:选取2015年3月—2016年1月在本院门诊就诊的子宫内膜容受性不足的不孕症患者100例,采用前瞻性队列研究的方法,根据是否接受中医温肾活血疗法将纳入的患者分为治疗组(52例)及对照组(48例),治疗组接受温肾活血法治疗及西医基础治疗,对照组只接受西医基础治疗。通过问卷调查,门诊复查,电话询问等方式进行随访,以子宫内膜厚度、类型、子宫动脉PI值及RI值、血清E2、P水平及妊娠率为观察指标,观察两组子宫内膜容受性改善的效果。结果:HCG日及黄体中期治疗组子宫内膜厚度明显高于对照组(P<0.05),治疗组A型内膜多于对照组(P<0.05)。治疗后两组子宫动脉PI值及RI值均明显降低(P<0.05),但治疗组下降幅度优于对照组(P<0.05);两组血清E2、P水平较治疗前均升高(P<0.05),组间比较E2、P水平差异有显著性(P<0.05)。治疗结束后治疗组妊娠率则显著高于对照组(P<0.05)。结论:温肾活血法能有效改善不孕症患者子宫内膜容受性,提高不孕症患者的妊娠率。Objective：To assess the effects of warming kidney and activating blood circulation method in improving endometrial receptivity for infertile patients.Methods：During March 2015 to January 2016,a prospective cohort study was employed.One hundred cases of infertility with low endometrial receptivity were enrolled and divided into treatment group（52 cases） and control group（48 cases） according to whether receiving traditional Chinese medicine warming kidney and activating blood circulation method,that is,treatment group was treated with warming kidney and activating blood circulation method as well as western basic therapy and control group was treated with basic therapy alone.A follow-up study was carried out by questionnaire,out-patient review and telephone interview.The effects on improving endometrial receptivity were observed mainly from endometrial thickness and type,uterine artery RI,PI,E2 and P levels and pregnancy outcome.Results：The thickness of endometrium in the treatment group was significantly higher than that in the control group（P〈0.05）.The type A endometrium in the treatment group was more than that in the control group（P〈0.05）.After treatment,the uterine artery RI and PI decreased significantly compared with those before treatment（P〈0.05）,but RI and PI in treatment group declined more significantly than those in control group（P〈0.05）.The E2 and P levels in two groups increased significantly after treatment（P〈0.05） and the level in treatment group was significantly higher than that in control group（P〈0.05）.There was a significant increase in clinical pregnancy rate in the treatment group compared with the control group after treatment.（P〈0.05）.Conclusion：Warming kidney and activating blood circulation method can effectively improve endometrial receptivity for infertile patients and enhance the pregnancy rate.
