龙蝎消水膏外敷联合腹腔热灌注化疗治疗恶性腹水的临床研究

摘　　要：目的:观察龙蝎消水膏外敷联合腹腔热灌注化疗治疗恶性腹水的有效性及安全性。方法:将76例恶性腹水患者随机分为治疗组(38例)和对照组(38例),治疗组采用龙蝎消水膏外敷联合腹腔热灌注化疗,对照组采用单纯腹腔热灌注化疗,观察两组腹水量、腹围、尿量、KPS评分、细胞免疫功能的变化;观察两组患者的中位生存期和6个月、9个月及12个月的生存率;观察两组患者的不良反应。结果:治疗组有效率(76.3%)高于对照组(47.4%),且差异有统计学意义(P<0.05);治疗后在缩小腹围、增加尿量、提高KPS评分、增强细胞免疫功能、延长生存期方面,治疗组优于对照组,且差异有统计学意义(P<0.05);两组不良反应均较轻,且差异无统计学意义(P>0.05)。结论:龙蝎消水膏外敷联合腹腔热灌注化疗可以有效控制恶性腹水,改善患者的生活质量,延长生存期,且不良反应轻,为恶性腹水的治疗提供了临床参考依据。Objective：To observe the efficacy and safety of Longxie Xiaoshui Ointment for external application plus hyperthermic intraperitoneal chemotherapy for patients with malignant ascites.Methods：Seventy-six patients with malignant ascites were divided into treatment group and control group randomly.The treatment group was treated by Longxie Xiaoshui Ointment for external application plus hyperthermic intraperitoneal chemotherapy.The control group was treated by hyperthermic intraperitoneal chemotherapy only.To observe the change of the amount of ascites,abdominal circumference,urine volume,KPS score and cellular immune function in two groups.To observe median survival time and the survival rates of six months,nine months and one year in two groups.To observe adverse reactions of two groups.Results：The effective rate of the treatment group（76.3%） was higher than that in the control group（47.3%） and the differences were statistically significant（P〈0.05）.In the aspect of reducing abdominal circumference,increasing urine volume,improving KPS score,enhancing cellular immune function and improving quality of life,the treatment group was superior to the control group after treatment,showing the significant difference（P〈0.05）.Two groups had slightly adverse reactions,showing insignificant difference（P〈0.05）.Conclusion：Longxie Xiaoshui Ointment for external application plus hyperthermic intraperitoneal chemotherapy has a good effect in both the treatment of malignant ascites and the improvement of the quality of life with slightly adverse reactions to provide the clinical reference basis for the treatment of malignant ascites.
