危重婴幼儿股静脉留置耐高压双腔PICC的应用研究

摘　　要：目的探讨危重婴幼儿股静脉留置耐高压双腔PICC导管的临床效果及并发症的发生率,为危重婴幼儿选取双腔导管建立有效的静脉通路提供临床依据。方法便利抽样选取我院PICU 2014年1月至2015年9月住院的PICC置管的危重婴幼儿140例,其中100例接受单腔PICC股静脉置管为对照组,40例接受双腔Power PICC股静脉置管为观察组,比较两组患儿的置管效果及导管留置期间并发症的发生率。结果观察组的出血量为(3.57±0.5)分高于对照组,差异有统计学意义(P<0.05);两组间操作时间、留置时间以及导管堵塞、导管相关血栓、导管相关感染、误入动脉的发生率等方面比较,差异均无统计学意义(均P>0.05)。结论股静脉留置耐高压双腔PICC导管安全可行,适用于危重婴幼儿使用。Objective To explore the clinical effect of the high pressure resistant double lumens PICC catheter in femoral vein of infants with serious illness feasibility,and the incidence rate of complication,in order to provide evidence for selecting double lumens PICC catheter in establishing venous access for critically ill infants.Methods By convenience sampling,140 cases were selected,in which 100 cases received single lumen PICC femoral vein catheter as control group,and 40 cases accepted the double lumens PICC femoral vein catheter as the observation group.The clinical effect and incidence rate of complications were compared in two groups.Results The bleeding amount in the observation group was（3.57±0.5）,which was lower than that in the control group（P〈0.05）.No statistical significance of cathetering time,indwelling time and the incidence rate of blockage of the catheter,catheter associated thrombosis,catheter related infection,misleading the catheter into artery was found in two groups（all P〈0.05）.Conclusion The high pressure resistant double lumens PICC catheter in femoral vein is a safe and feasible device for the critically ill infants.
