小切口超薄真皮下血管网皮瓣联合MEBO治疗腋臭疗效观察

摘　　要：目的观察小切口超薄真皮下血管网皮瓣联合湿润烧伤膏(moist exposecd burn ointment,MEBO)治疗腋臭的临床疗效。方法对2009年1月-2016年9月山西省运城市盐湖区东城薛大夫整形外科门诊部及山西医科大学第二医院整形外科收治的66例腋臭患者实施小切口超薄真皮下血管网皮瓣联合MEBO换药治疗,观察治疗效果。结果所有患者均无皮瓣坏死,除2例患者出现单侧皮下血肿行穿刺抽吸术及1例患者出现单侧皮下血肿拆除1针缝线清除后愈合外,其余患者切口均一期愈合,且愈后皮肤均无明显瘢痕增生;随访3个月,所有患者均无上肢功能障碍,腋臭分级为0级者60例,1级者6例。结论小切口超薄真皮下血管网皮瓣联合MEBO治疗腋臭,术后并发症较少,切口愈合较好,疗效满意,值得临床推广应用。Objective To observe the clinical efficacy of small incision ultra-thin subcutaneous vascular network flap （SVNF） in combination with Moist Exposed Burn Ointment （MEBO） in the treatment of axillary osmidrosis. Methods Sixty-six patients with axillary osmidrosis admitted to Dr. Xue＇ s Plastic Surgery Clinic and Plastic Surgery Department of The Second Affiliated Hospital of Shanxi Medical University were treated with the small incision ultra-thin subcutaneous vas- cular network flap in combination with the dressing change therapy of MEBO. The clinical outcomes were then observed. Results No flap necrosis was observed in all patients. Except that one-side haematoma occurred in 2 patients on whom per- cutaneous needle aspiration was performed, and another one patient with one-side haematoma underwent the removal of one stitch followed by aspiration, all the other patients achieved primary healing without obvious hypertrophic scars. The 3-month follow-up showed no functional impairment of upper limbs, and 60 patients were grade 0 of body odour while 6 pa- tients were grade 1. Conclusion Treating axillary osmidrosis with the small incision ultra-thin subcutaneous vascular net- work flap in combination with MEBO can reduce post-operative complications and promote the healing of incision wound, which should be promoted in clinical practice.
