补肾活血汤治疗股骨颈骨折术后延迟愈合临床疗效研究

摘　　要：目的:探讨补肾活血汤及护理治疗股骨颈骨折术后延迟愈合的临床疗效。方法:选取我院骨科收治的股骨颈骨折术后延迟愈合患者144例,按随机数字表法分组,对照组72例予以自体骨髓移植治疗,研究组72例在对照组基础上予以补肾活血汤治疗,观察并记录两组间髋关节功能评分,骨代谢指标水平,血清s ICAM-1、IGF-1、NGF水平,同时比较临床疗效及并发症发生率。结果:对照组总有效率(86.11%)低于研究组总有效率(95.84%),具有统计学意义(P<0.05);与对照组比较,研究组治疗后疼痛、功能、肢体畸形、关节活动度Harris评分较高,治疗后血清ALP、BGP、BMP-2水平较高,治疗后血清s ICAM-1水平较低,血清IGF-1、NGF水平较高,具有统计学意义(P<0.05);对照组并发症发生率(16.66%)高于研究组(5.66%),具有统计学意义(P<0.05)。结论:补肾活血汤治疗股骨颈骨折术后延迟愈合的临床疗效较好,能明显提高患者髋关节功能,改善骨代谢指标,降低血清s ICAM-1水平,提高血清IGF-1、NGF水平,促进骨折愈合,减少并发症。Objective ：To investigate the clinical effect of Bushen Huoxue Decoction in delayed healing of patients with femoral neck fractures after operation. Methods ：144 delayed healing of patients with femoral neck fractures after operation from the department of orthopedics in our hospital were selected and divided into 2 groups,72 cases in the control group treated with autologous bone marrow transplantation,72 cases in the experiment group treated with Bushen Huoxue Decoction on the basis of the control group,hip function score,bone metabolism index levels,serum s ICAM-1,IGF-1,NGF levels,and the clinical effect and complication rate were compared after the treatment. Results ：The total effective rate in the control group（86.11%）was lower than that in the experiment group（95.84%）,with significant difference（P〈0.05）;compared with the control group,Harris scores of pain,function,limb deformity and joint mobility were higher after treatment in the experiment group,the serum levels of ALP,BGP,BMP-2 were higher after treatment,the serum level of s ICAM-1 was lower after treatment,the serum levels of IGF-1,NGF were higher after treatment,with significant difference（P〈0.05）;the complication rate in the control group（16.66%）was higher than that in the experiment group（5.66%）,with significant difference（P〈0.05）. Conclusion ：The clinical effect of Bushen Huoxue Decoction in the treatment of delayed healing of patients with femoral neck fractures after operation is good,can significantly improve the hip function,improve bone metabolism index,reduce the level of serum s ICAM-1,improve the level of serum IGF-1 and NGF,promote fracture healing,and reduce complications.
