原发性高血压患者血压变异性与颈动脉粥样硬化斑块的关系

摘　　要：目的研究原发性高血压患者血压变异性与颈动脉粥样硬化斑块的关系。方法选取23例原发性高血压患者(高血压组)及合并颈动脉粥样硬化斑块的16例原发性高血压患者(合并硬化组)为研究对象。比较2组血压变异性、血脂及颈动脉内膜中层厚度,采用Spearman相关分析血压变异性与颈动脉内膜中层厚度的相关性。结果合并硬化组血压变异性指标及颈动脉内膜中层厚度大于高血压组(P<0.05),2组血脂指标比较差异无统计学意义(P>0.05)。颈动脉内膜中层厚度与白昼、夜间、24h舒张压标准差及收缩压标准差均呈正相关(r值分别为0.609、0.468、0.712、0.746、0.554、0.597,均P<0.05)。结论原发性高血压患者颈动脉粥样硬化的发生与血压变异性密切有关,对其进行监测,能为疾病的防治提供参考。Objective To study the relationship between blood pressure variability and carotid atherosclerosis in patients with essential hypertension.Methods Blood pressure variability,blood lipid and carotid intima-media thickness were measured in 23 patients with essential hypertension（hypertension group）and 16 patients with essential hypertension complicated by carotid atherosclerotic plaque（complicated atherosclerosis group）.The correlation between blood pressure variability and carotid intima-media thickness were analyzed by Spearman correlation analysis.Results Compared with hypertension group,blood pressure variability and carotid intima-media thickness significantly increased in complicated atherosclerosis group（P〈0.05）.There were no significant differences in blood lipid levels between the two groups（P〉0.05）.The carotid intima-media thickness was positively correlated with daytime,nighttime and 24-hour diastolic blood pressure standard deviation and systolic blood pressure standard deviation（r=0.609,0.468,0.712,0.746,0.554 and 0.597,respectively;P〈0.05）.Conclusion The occurrence of carotid artery atherosclerosis is closely related to blood pressure variability in patients with essential hypertension.Therefore,the monitoring of blood pressure variability can provide a reference for disease prevention and treatment.
