摘  要:目的研究糖化血清清蛋白(GA)与糖化血红蛋白(HbA1c)的相关性,比较两者在糖尿病(DM)诊疗中的临床应用价值。方法选取2015年3-8月在重庆市第七人民医院进行DM初筛人群290例作为观察组,选择同期体检的健康体检者50例作为对照组。将观察组分为糖耐量正常(NGT)组、单纯空腹血糖受损(IFG)组、单纯糖耐量异常(IGT)组、空腹血糖受损和糖耐量异常(IFG/IGT)组、DM组5个组。行口服葡萄糖耐量试验(OGTT),采用液态酶法测定GA水平,采用高效液相色谱法测定HbA1c水平,采用SPSS17.0统计学软件分析检测结果。结果 DM组GA、HbA1c、空腹血糖(FPG)、餐后血糖(PPG)与其他4组及对照组间比较差异有统计学意义(P<0.05);NGT组与IFG/IGT组血清GA水平差异有统计学意义(P<0.05)。GA水平与HbA1c具有明显相关性。回归方程Y=0.230 5 X+2.221 8,R2=0.744 6。HbA1c与GA在各组的阳性检出率比较中,GA与HbA1c在NGT组和糖耐量减低组差异均无统计学意义(P>0.05),在DM组差异有统计学意义(P<0.05)。结论 GA是反映近期血糖变化的良好指标,与HbA1c有良好的相关性。Objective To investigate the correlation between glycosylated serum albumin(GA)and glycosylated hemoglobin(HbA1 c)and to compare the clinical value of the two methods in the diagnosis and treatment of diabetes.Methods Methods a total of 290 cases of diabetes screening in our hospital from March 2015 to August 2015 were selected as the observation group,and the same period in our hospital for health examination of the 50 cases of physical examination as the control group.The observation group was divided into 5 groups:normal glucose tolerance(NGT)group,pure fasting glucose impaired(IFG)group,pure glucose tolerance(IGT)group,impaired fasting glucose and glucose tolerance disorder(IFG/IGT)group and diabetes mellitus(DM)group.The oral glucose tolerance test(OGTT)was carried out.The GA was determined by liquid enzyme method and HbA1 cwas determined by HPLC.The results were analyzed by SPSS17.0 statistical software.Results DM group GA,HbA1 c,FPG,PPG and the other four groups and the difference between the control group was statistically significant(P〈0.05);NGT and the control group and IFG/IGT group serum GA was statistically significant(P〈0.05).GA and HbA1 chave significant correlation.Regression equation Y=0.230 5 X+2.221 8,R2=0.744 6.Compared with the positive rate of HbA1 cand GA in each group,GA and HbA1 cwere not significantly different between the normal glucose tolerance group and the control group(P〈0.05),and there was significant difference in the diabetic group(P〈0.05).Conclusion GA is a good index to reflect the recent changes in blood glucose,which has a good correlation with HbA1 c.
