地佐辛预防腹腔镜全麻术后导尿管相关性膀胱刺激征的效果观察

摘　　要：目的观察地佐辛预防腹腔镜全麻术后导尿管相关性膀胱刺激征(catheter-related bladder discomfort,CRBD)的效果。方法择期行腹腔镜胃癌根治术的患者40例,ASAⅠ或Ⅱ级,随机分为地佐辛组(D组)和对照组(C组)各20例,分别在手术结束前20min静脉注射地佐辛0.1mg/kg或生理盐水2mL。记录2组手术时间、苏醒和拔管时间;观察手术结束时(T1)、拔管时(T2)、拔管5min(T3)、拔管10min(T4)患者的MAP、HR、和SpO_2;在麻醉恢复室评估CRBD的严重程度和Ramsay镇静评分以及不良反应发生率。结果 2组MAP、HR均呈先升高后降低趋势,SpO_2是降低趋势,2组在组间、时点间、组间·时点间交互作用差异均有统计学意义(P<0.05)。D组CRBD严重程度显著低于C组(P<0.05),Ramsay镇静程度优于C组(P<0.05)。结论地佐辛可有效预防腹腔镜全麻术后CRBD,且血流动力学平稳。Objective To evaluated dezocine for preventing catheter-related bladder discomfort(CRBD)after laparoscopic surgery in general anesthesia.Methods Forty patients(ASA ⅠorⅡ)undergoing laparoscopic radical gastrectomy were randomly divided into two groups:dezocine group(group D)and control group(group C),20 cases in each.Group D received dezocine 0.1 mg/kg,or group C received normal saline 2 mL 20 min before the end of operation.The operation time,the consciousness recovery time and extubation time were recorded.The MAP,HR and SpO2 were recorded after the surgery(T1),during extubation(T2),5 min(T3)and 10 min after extubation(T4),respectively.In the post-anesthesia care unit severity of CRBD and sedation scale was assessed,the incidence of the adverse reaction were also compared.Results The two groups of MAP and HR were all increased first and then decreased,and SpO2 was downward trend.MAP,HR and SpO2 had statistically significant differences in two groups compared with groups,different time points and groups·time points(P<0.05).The severity of CRBD in group D was reduced significantly compared with those in group C(P<0.05),Ramsay sedation scale at extubation later was better in group D than that in group C(P<0.05).Conclusion Dezocine can effectively prevention CRBD after laparoscopic surgery in general anesthesia and maintain hemodynamic stability.
