腹部联合肛周穴位按摩预防重症脑卒中大便失禁患者失禁性皮炎的效果观察

摘　　要：目的探讨腹部联合肛周穴位按摩预防重症脑卒中大便失禁患者失禁性皮炎的效果。方法采用便利抽样法,选取2016年2—12月本院重症监护室收治的重症脑卒中60例患者作为研究对象。按随机数字表法分为观察组(n=30)与对照组(n=30)。对照组予常规护理,观察组在常规护理基础上予腹部联合肛周穴位按摩,2次/d,持续7 d。比较2组患者干预后失禁性皮炎发生率、发生时间及发生后严重程度。结果干预后,观察组患者发生失禁性皮炎4例,发生率为13%,失禁性皮炎发生时间(4.68±0.89)d,失禁性皮炎严重评分(4.07±0.85)分,均低于对照组(P<0.05)。结论腹部联合肛周穴位按摩有助于延缓失禁性皮炎发生时间,减轻失禁性皮炎发生的严重程度,可有效预防重症脑卒中大便失禁患者失禁性皮炎。Objective To investigate the effects of abdominal and crissum acupoint massage on promoting patients＇ regular and concentrated defecation to prevent incontinence-associated dermatitis（IAD） in patients with severe stroke. Methods Sixty patients with severe stroke in ICU from February to December in 2016 were randomly divided into observation group and control group with30 cases in each group. Control group received routine nursing while observation group additionally abdominal and crissum acupoint massage for seven consecutive days and twice per day. The incidence rate, occurrence time and severity of IAD were compared between two groups after the intervention. Results After the intervention, the incidence rate of IAD of observation group was 13 %;occurrence time 4.68 ±0.89 days and IAD severity score 4.07 ±0.85. All the differences were significant（P0.05）. Conclusion Abdominal and crissum acupoint massage can reduce the incidence and severity of IAD in patients with severe stroke and it can effectively prevent the occurrence and progression of IAD.
